Is the workplace clean and orderly?

Proper Stacking of stored material

Are floors cleared of tripping hazards (electrical/phone cords, loose carpeting,


miscellaneous objects in walkway, etc.)?

Waste bins are available and labelled


Timely collection of Waste

Everyone know how to submit accident forms or report safety concerns?

Everyone know how to respond to emergencies (Fire, Medical, etc.)?

Personal Protective Equipment

Are all the required PPEs available in the area?

Are workers wearing the provided PPE?

Are appropriate signs of PPE on the entry points of an area?

Any damaged PPE in the area

Fire Safety

Are emergency exit signs visible and lit?

Are all emergency exits clear of obstructions and accessible?

Are fire extinguishers visible and accessible?

Are all exitways clear of obstructions that would prevent a quick and safe exit in the event
of an emergency?

Electrical Safety

All the electrical boards/panels are locked or access control

Electrical Resistive mats present with every electrical DB

Any loose/open wiring in the area (if yes, mention location in comments)

Are power strips plugged directly into a wall outlet? Extension cords are to be used for
temporary applications only (one day or less).

Are grounded plugs (three-prong) connected into a grounded outlet or grounded


extension cord/power strip?

Chemical Safety

Are all hazardous chemicals (toxic, flammable, reactive, corrosive) stored properly? Are
spill kits available for chemicals storage?

Are hazardous chemicals labeled properly (name, hazard, cautions, etc.)?

Are Material Safety Data Sheets (MSDS) available for all hazardous chemicals (excluding
“consumer grade” chemicals)?

Are unneeded or outdated chemicals disposed of properly (not down the sink or in the
general trash)?

Ergonomics

Are workers aware of safe lifting techniques?

Do they perform tasks that are physically repetitive?

Do they take regular breaks from repetitive-motion tasks?

Are tools, instruments and machinery shaped, positioned and handled so those tasks can
be performed comfortably?

Is workstation furniture adjusted, positioned and arranged to minimize strain on all parts
of the body?
Machine Safety

Are all dangerous parts of the machinery guarded?

Are all guards of good construction, adequate strength and well maintained?

No guards are bypassed

Do guards permit an adequate view of the operation where this is necessary?

Is appropriate protective equipment available (gloves, eye protection, etc.) and used when
working with chemicals?
