REMITTANCES

Greetings!

In view of the AGENCY ACTION PLAN STATUS OF IMPLEMENTATION (Audit

Observations and Recommendations C.Y. 2016) of our resident auditor recommends to adopt
the automated bank scheme in paying our HDMF Remittances and also the PAG-IBIG
FUND AGENCY required us to comply. In line with this, we are requesting for the We
Access Enrollment of our HDMF Remittances, otherwise they will not receive our
remittances in lieu of cash or check. Ms. Johaira M. Faisal will be the maker and Ms. Faidah
S. Pangandaman will be the authorizer, if the said request will be approved by your bank.

We are hoping for your benevolent consideration on this matter.
