PROJECT: - Ammonia DBN Project Mechanical, ELectrical & Instrumentation Works

Expalanatory Notes
(#1) FFL will supply low alloy Electrodes only

FFL will provide power at one point in Cooling Tower, Ammonia Plant, Site workshop & Labour camp (If any). This power will be provided without Distribution

board & within 100 M range of above mentioned areas.

(#3) FFL will provide construction water only i.e. for Hydrotest & Flushing purpose only.

FFL will provide free land for labor camp for Contractor workers. Contractor to prepare labor camp at his own cost. Contractor to arrange accomodation services

for his management staff. FFL will not provide any accomodation services for Contractor Management staff.

(#5) FFL will identify the scrap/debris disposal place within FFL boundry limit.
(#6) FFL will identify route for Contractor manpower entry in to plant permises.
FFL will examine the Walkie Talkie sets arranged by Contractor. FFL will share particular bandwidth to contractor. Contractor to ensure using FFL mentioned

bandwidth only.
FFL will provide the safety orientation of Contractor every employees. Contractor to ensure that none of his employees enters plant without safety orientation by

FFL.

(#9) FFL will arrange training course for Contractor Scaffolding lead who will only certify scaffoldings erected by Contractor.

(#10) Safety waiver can only be provided for Cooling Tower area subject to FFL Management approval.
(#11) Contracor to ensure addequate manpower according to the project requirement.
(#12) Contractor to incorporate the observations of Owner
Contractor will critically review the detailed engineering drawings for any short comings and shall inform Owner for rectification.

(#14) The approval of the Owner will not diminish Contractor's responsibility
(#15) Resposibility of Owner and Contractor as per mutually agreed / signed document
Fatima is responsible for general security of its Premisis. Conflicting Security Intersts must be clarified by Contractor with Fatima to avoid shift of responsibility
(#16)

(#17) Contractor and Owner will mutually agree on the point of supply and conditions thereof.
(#18) Contractor to discuss detailted procedures for disposal to designated site with Owner
(#19) The implications of repair work on Performance and Guarantees to be agreed with Owner
(#20) Owner or Contractor may appoint Third Party Inspectors as required, however all payment to third party will be on Contractor account

1- Project Scheduling
• Overall Project completion time for all projects. X

2- Documentation / Dossiers

3- Material Requirement

4- Resource Planning

5- Job Execution Scope

• No Subcontracting is allowed for any type of work without written consent of Fatima A X

• Repair of any leakage observed from Existing Equipment caused by Contractor shall be rectified A X

• Permit to work system shall be followed by Contractor for all jobs unless specified in JSA or HSE waiver. X

• Contractor to ensure that operating areas must be separated by Hard barrication with corrugated sheets from construction activities. R X

• Contractor to ensure Blowing/Flushing of Pre-fabricated spools in the workshop (Pre-fabrication yard) before shifting to actual location in relevant plant

6- Inspection - QA/QC

7- Safety
•All contractor’s workforce must go through safety Induction prior to start work at site. X X Note # 8

• Contractor shall provide desired PPE's e.g. safety helmets, safety shoes, goggles, fire
blanket (if required) & other safety items to all manpower working for the execution X
of project.

• Contractor shall provide sample PPE's i.e. safety helmets, safety shoes, goggles, fire

blanket (if required) for Fatima approval.

• Double Safety harness for all jobs above 6 Ft height. Face shield for all cutting/grinding jobs X

• No work to be carried out without the proper PPE's & authorized signed work permits. X
• Work permit to be displayed always at the work place during execution. X
• All electrical equipment for the execution of job will be inspected & approved by

• Contractor HSE Supervisor shall be available for maximum 50 person in the Construction area (Cooling Tower) X

• Contractor HSE Supervisor shall be available for maximum 20 person in the Operating area X

• No work at height unless erected scaffolding is inspected and Green Tagged by Contractor scaffolding inspector. X

• Contractor will nominate Scaffolding inspectors. Fatima shall conduct their training & certify them. X X Note # 9

• Contractor to bring experienced Scaffolders. Scaffolders must be certified by Fatima R X

8- Others

• Loading and unloading of the materials to be done carefully to avoid any damage. X

• Complete housekeeping during & after completion of the job. Contractor shall perform housekeeping on Daily basis. The removed material i.e. Debris
or scrap shall be disposed by Contractor according to their designated places or advised by Fatima concerned Engineer. X

• The work has to be done up to the entire satisfaction of the FATIMA's Engineer. R X
