INTRODUCTION

Dear ULB User,

Welcome to Safaimitra Suraksha challenge


As you are aware, for Safaimitra Suraksha challenge, you will be required to upload PDFs or Excel (as per requirement of
the indicator) and/or scanned version of supporting documents against each of the indicators that you are seeking
marks for.

Document’s checklist is already shared with you and for reference we have mentioned all points and formats in this
manual. However, if the ULB is maintaining the formats/documents that carry the relevant information that are
mentioned in indicators, they can also upload them.

We sincerely hope that all the ULBs will upload documents with full enthusiasm to maximize their marks and make
Safaimitra Suraksha challenge a grand success.

Indicator 2.1 Document 1
Complaint Reports till 30th April, 2021
Points that will be checked in this document are as follows:

• Complaint report should be submitted month wise


• The report should indicate the type of complaint registered
• Complainant’s Name (Optional), Phone Number should be mentioned
• Date and time of complaint opening and closing should be mentioned
• Each redressal system should have mechanism for generating monthly report showing all
complaints received, in process, resolved and feedback received.

List of complaints along with numbers from various mediums like helpline, Portal, WhatsApp

Points that will be checked in this document are as follows:

• Complaint report should be submitted month wise


• The report should indicate the type of complaint registered
• Medium on which the complaint was received should be specified
• Complainant’s Name (Optional), Phone Number should be mentioned
• Date and time of complaint opening and closing should be mentioned
• Each redressal system should have mechanism for generating monthly report showing all

• Please provide helpline number, WhatsApp number and portal link for complaints

• Need copy of agreement in place at ULB level for non-compliance in resolution time
• Copy of escalation matrix followed by the ULB

Evidence of mechanism in place to ensure customer is satisfied before closing the complaint.

Points that will be checked in this document are as follows:
