AUTHORIZATION

This is to authorize Ms. Myla Joyce Lumabao to obtain my documents or related

credentials on my behalf at Technological Institute of the Philippines Manila (TUP)-Office of
Registrar. These documents are required for my application to 2021 Department of Science and
Technology-Junior Level Science Scholarship (DOST-JLSS) in which the deadline of
application is on October 29, 2021. The reason I am not able to get my credentials personally is
because I am currently in Tarlac province, in my permanent address, and it is hard for me to
travel because of the government restriction on travel during the imposed community quarantine.
