Important instructions for candidates


1. As a precaution due to Kovid-19, the candidate has to reach the center on the above mentioned reporting / entry time.
2. No candidate will be allowed to enter after the time the gate is closed.
3. No candidate will be allowed to leave the examination hall / hall before the examination ends.
4. Upon completion of the examination, please wait for the instructions of the inspector and do not get up from your seat unless told.
Only one candidate will be allowed to go out at a time.
5. All the candidates have to download and carefully read the instructions and advice of Kovid-19 given with the admit card and follow
them strictly.
6. This admit card and related details are in four pages - page 1 contains center details and self declaration (undertaking) form related to
Kovid-19, page 2 contains "Important Instructions for Candidates" and pages 3 and 4 "Advice for Candidates Related to Kovid-19". The
candidate has to download all four pages.
7. The admit card is provisional, subject to meeting the eligibility conditions as given in the information bulletin.
8. Candidates are suggested to visit the examination venue a day in advance so that they do not face any problem on the day of
examination.
9. If you are required to wear speci c dress according to religion / custom, please arrive at the center early for a complete investigation.
10. No candidate will be allowed to enter the examination center without the admit card, valid ID proof and proper Frisking.
11. Candidates are advised to bring only the following items to the examination venue / center:
  a) Self declaration (Undertaking) downloaded from the SAMS website (clear printout on A4 size paper) along with the admit card
which is duly Must be lled.
  B) Government issued ID identity proof
  c) Two simple transparent ball point pens
  d) Personalized hand sanitizer (50 ml)
  e) Individual transparent water bottle
12. Before arriving at the center, candidates must enter the required details in the Undertaking in legible handwriting and af x their
photograph to the appropriate place.
13. Candidate should get "any one of the original and valid photo identity proof issued by the government" - PAN Card / Driving License /
Voter ID / Passport / Aadhaar Card (with photo) / E-Aadhaar / Ration Card / Photo The Aadhaar enrollment number will have to be
brought along. All other IDs even if attested (scanned) / scanned photo of ID in mobile phone will not be considered as valid ID proof.
14. Candidates are not permitted to take any personal items including electronic devices, mobile phones and other restricted / prohibited
items listed in the information bulletin to the examination center. The examination of cer will not be responsible for the safe custody of
personal belongings and there will be no such facility.
15. Two additional carbon copies of OMR sheets (copy of NHM and copy of candidate) and question paper will be provided in the
examination hall / room along with the answer book (OMR sheet). Before exiting the examination hall / room, the candidates have to ll
all the necessary details in the answer book and submit the original OMR lled with admit card, copy of NHM, OMR sheet, question
paper and admit card to the inspector.
16. At the end of the examination, the duly lled admit card should be handed over to the complainant.
17. No candidate should adopt any unfair means nor engage in any unfair examination method.
18. Candidates are advised to regularly check updates on the SAMS website. For the latest updates and information, keep checking SMS

Notice for the candidates regarding Kovid-19

SAMS will implement social distance measures as per the guidelines of the Government of India / Uttar Pradesh in the current scenario of
Kovid-19 to ensure the health and safety of the candidates. Adequate measures have been implemented to protect everyone without
compromising high standards, purity and impartiality in conducting the examination. Candidates are required to follow the guidelines and
the new procedure for social distance and hygiene to ensure the safety and health of their own and fellow candidates. For security
purposes, SAMS strongly advises candidates not to bring anything other than permitted items.
Center preparation
1. Standard operating procedures are being implemented to implement safety precautions and maintain necessary standards for hygiene.
2. The seating area will be thoroughly cleaned before the examination.
3. The distance between 2 seats will be maintained as per the guidelines.
4. Attempts will be made to provide hand sanitizers to candidates and center staff to use, but they will be urged to bring their own
sanitizer.
5. An attempt will be made to ensure that all procedures are touch-free to ensure social distance norms.
6. Candidates will have to arrive at the center as per reporting / entry time at the center given in the admit card to avoid any congestion at
the center at the time of admission and to maintain social distance.
Pre-exam preparation
1. Candidate should know the reporting / entry time at the center given in the admit card and arrive at the center only according to the
reporting time so that any congestion at the center at the time of admission can be avoided and social distance maintained.
2. Candidate should ll the admit card completely and properly as per instructions.
3. Candidates will be allowed to take only certain items with them to the examination venue:
a) Self declaration (Undertaking) downloaded from SAMS website (clear printout on A4 size paper) with admit card which should
be duly lled .
B) Government-issued ID identity proof
C) two simple transparent ball point pens
d) individual hand sanitizer (50 ml)
e) individual transparent water bottle
At the time of entry
1. Candidates will bring their face mask and wear it at all times during the examination. Make sure the mask ts to cover your nose,
mouth and chin. No face mask will be allowed to enter the center.
2. Candidates must maintain a distance of at least 6 feet from each other at all times. Floor marks will be arranged outside the center,
follow the instructions given by the staff of the center.
3. Candidates will have to wash their hands with soap and clean their hands with hand sanitizer before entering the center. Efforts will be
made to provide hand sanitizers at various locations of the center.
4. The candidate has to bring the duly lled admit card as per the instructions.
5. Candidate will have to bring the identity proof and necessary documents as indicated under points 11 and 13 of the important
instructions for the candidates given on page-2 .
6. Do not bring prohibited items to the examination center as there is no system available to keep your belongings safe. Please refer to
point-11 of important instructions for candidates given on page-2 for permitted items .
7. At the time of admission, lled undertaking on the admit card, and body temperature (using thermo guns) will be checked and the
center staff will guide you to the respective room / hall. Candidates are required to strictly follow the instructions provided by the
employees of the center.
8. Candidates with fever, cough and cold symptoms will be asked to undergo rapid antigen test (it will be arranged at all centers) before
the examination. Candidates whose report will be positive will be debarred from taking the exam as per the signed declaration while
downloading the e-admit card.
9. Please note that no candidate will be prohibited from appearing in the examination. However, if a candidate violates the Central / State
Government's COVID-19 instructions / advice and instructions mentioned in the admit card applicable on the day of examination, then
such a candidate may be denied the examination.
