GENERAL INFORMED CONSENT FORM

INTRODUCTION
As a research institution, De La Salle University commits itself to the generation and
propagation of new knowledge for human development and social transformation. The Senior
High School Research Curriculum is a venue for our students to be part of this commitment. By
demonstrating the necessary knowledge, skills and emotional disposition, students will
conceptualize, design, validate and disseminate their research ideas and results to address an
identified societal challenge, need or opportunity.
The conduct of research in the University is governed by a system of moral principles that
aims to maintain the highest standards of duty, professionalism and quality. These principles
constitute the code of ethical research practices that guides and informs the pursuit of knowledge
towards improving the quality of life in Philippine society. Among these principles include the
safety and protection of both the research proponents and the test subjects/participants. The SHS
Department upholds the above principles by preparing parental informed consent and participant
informed assent forms and requiring such to be accomplished before the commencement of any
research or study.
A number of research ideas from our students are geared towards understanding the
different facets of Senior High life as experienced by their peers. Thus, the DLSU Senior High
School students may be invited by their peers to participate in some of these studies.
The purpose of this form is twofold. First, it gives you a general overview of these studies,
their procedures for data collection, the data to be collected, and confidentiality clauses. Second,
it is a means to seek your consent in allowing your children to participate in some of these studies.
Kindly read and understand the information provided. Feel free to ask any questions about
the information that is unclear to you. You may also discuss the information with your children
before you reach a decision.
RESEARCH DESCRIPTION
The research topics conceptualized by our Senior High School students range from art
appreciation, computer technology, education, the environment, health and psychology. The
complete list is found in the attachment. In general, these studies seek to determine and explore
the different factors that are influencing and affecting the Senior High students.
Each of these studies may need at least 40 volunteer participants from whom data need
for analysis will be collected. Every study is expected to produce a research paper, which
describes the purpose and rationale, the procedure employed in collecting data, the findings and
subsequent recommendations.
DATA COLLECTION PROCEDURE
Different data collection strategies may be employed, specifically, one-on-one interview
with any research team member, accomplishment of paper or online survey forms, and group
discussions. Each data collection session may last from 15 minutes to, at most, an hour. Some
may require the participants to log daily activities, while others will conduct multiple data collection
sessions. The interviews may be recorded to ensure that the information provided by the
participants are properly recorded and documented. Video recordings and pictures will only be
taken as needed, and with proper advice given to the participants, prior to soliciting their consent.
All collected data and recordings will only be used for analysis and will not be uploaded in any
social media sites. Anonymity and confidentiality will be ensured at all times by not recording the
identities of the participants.

VOLUNTARY PARTICIPATION
Participation in any study is voluntary. You may decline if you do not want your child to
participate in any of the studies conducted by her or his peers.
Should you decide to give your consent to allow your child to participate in any of the studies,
you will be requested to sign the Informed Consent Form attached to this document. Upon signing
the form, it is considered that:
✓ You fully understood what you have read;
✓ You allow your child to participate in any of the studies conducted by her or his peers;
and
✓ You allow the research team to use the information to be provided by your child in their
analysis.
Please note that your child will also be accomplishing an Informed Assent form for each of
the studies she or he may wish to participate in. Your child will also be given an opportunity to
ask questions, to voluntarily take part in the study, to stop the interview and survey any time, and
to withdraw from the study.
Your consent to allow or not allow your child to participate in any of the studies will not affect
her or his grades in any of her or his classes in the Senior High School program.
BENEFITS AND RISKS
There are no direct benefits to your child for taking part in the studies. But your child’s
participation will help her or his peers in achieving part of the objectives of their studies, as well
as, indirectly contribute to knowledge generation. Some studies may lead to a proposal on
intervention programs and strategies that may improve the home and school environment of your
child and her or his peers.
All studies to be conducted by our Senior High School students have been screened by
faculty mentors, advisers, and researchers in the University. Ethics review has also been
conducted to ensure that the research methodology will observe proper procedures when dealing
with human participants, vulnerable sectors of society, infectious agents, toxic substances, and
public data.
CONFIDENTIALITY
All data to be gathered from the participants will be kept private. All personal information
will be kept confidential. Real names will not be used. Hence, each participant will be assigned
an alias or reference code. Quantitative data will be aggregated; while all interviews, surveys and
discussion will be presented in a summarized format to ensure that the participants’ identities are
kept anonymous and confidential. All information collected from the studies that are written and
recorded will be destroyed after five (5) years.
