AFFIDAVIT OF CONSENT TO TRAVEL

I, RIZZA AIZA PAGADUAN COLLADO, of legal age, Filipino citizen, single,

and with residence and postal address at Bautista, Caba, La Union, after having been duly
sworn to in accordance with law, do hereby depose and say:

1. That I am the mother of LA REINA HAZER COLLADO PEŃA, who was

born on 15 March 2010 in La Union Medical Center (LUMC), Agoo, La Union;

2. That I am giving my consent for my child to travel to Alaska, U.S.A. and

accompanied by her step-mother, SHARMAINE FLORES PEŃA;

3. That the child will leave the Philippines on March 10, 2021 and will stay in the
said place of destination for a long period of time and will be taken care of her
father, ZOSIMO ESPEJO PEŃA and her grandmother, LUZVIMINDA PEŃA;

4. That I execute this affidavit for whatever legal purpose it may serve in
connection with my child’s travel to Alaska, U.S.A. on March 10, 2021.
