Procedure FOR STUDENT CLEARANCE

 Fill out completely the Student's Clearance Form.


 Once the student has been cleared, the Registrar's Office will email an electronic copy of the
grades.

Procedure ON ONLINE ENROLLMENT


 If the student is 4th year standing, submit a letter requesting for an evaluation to
registrar@olopsc.edu.ph prior to enrollment.
 Once received, registrar’s staff will evaluate the student to verify his/her standing, subjects
taken, lacking subjects, etc.
 Registrar’s staff will send the evaluated curriculum to Student cc Program Director/Dean.
 Student can proceed to enrolment by clicking the link provided and select type of Application.
 Fill out the College Enrollment and Assessment Form via Google form.
 Select subjects to be enrolled.
 Submit form for review and assessment by the Program Director/Dean and approval by the
College Registrar.
 Wait for email notification from the Registrar's Office.
 Email a clear photo or scan of proof of payment to collegecashier@olopsc.edu.ph with the
subject line: PROOF OF PAYMENT: [COURSE/STUDENT NAME]

Procedure ON SUMMER ENROLLMENT


 Completely fill out the Summer Enrollment Form via Google form and attach the Letter of Intent
(LOI) as part of the registration process.
 Once submitted, the Program Director will assess and determine the availability of course
offerings and the faculty to handle the loading.
 The Program Director will endorse the requested course enrollment to the Dean.
 Once approved, the Dean coordinates with the Registrar's Office.
 The Registrar's Office will notify the applicant via email to proceed with the enrollment.

Procedure ON LOAD REVISION


 Click the link provided and fill out the LOAD REVISION FORM via Google form.
 Select the type of request (Add/Change/Drop).
 Submit form for review and assessment.
 Approval of revision requests will be sent via email.
 The student must accomplish the Load Revision Form within the deadline set by the Office of the
Registrar.

PROCEDURE IN APPLYING FOR DEAN’S LIST


 Send a Letter of Intent with attached clear copy of Report of Ratings addressed to the College
Dean at deansoffice@olopsc.edu.ph on or before July 15, 2020.
 The College Dean will make a recommendation and submit all the documents to the Office of
the Registrar for validation and processing.

GENERAL POLICY
 A student who obtains a general weighted average (GWA) of at least 1.75 at the end of each
semester qualifies for the Dean’s List provided that he/she:
o has load of not less than 18 units
o has not incurred a grade lower than 2.00 or a mark of IP, WP, FWP, FA or NA in any of
the subjects
o has not committed any misdemeanor or immoral conduct during his/her stay in College.
 A student who is included in the Dean’s List for six (6) consecutive semesters shall be
automatically considered as a candidate to receive an academic honor.
 A Dean’s Lister receives a corresponding discount on the tuition fee only and a merit card in the
next academic year during the Annual Students’ Assembly.
