AFFIDAVIT

I, ZARAH DENISE O. ROSOS of legal age, single,

Filipino, with residence and postal address at
Baliwasan, Zamboanga City, Philippines, after
having been duly sworn to an oath in accordance
with law, do hereby depose and say that:

1.I work as a Legal Assistant at the Law Offices

of Michael P. Molina located at No. 64 St.
Michael’s, Estrada Street, Tetuan, Zamboanga
City, Philippines 7000;

2.My work involves carrying out a variety of

day-to-day functions in the law office such as
receiving clients, doing research work,
litigation monitoring, and filing of pleadings
and documents;

3.Last January 20, 2017, I have filed a document

entitled “Petition” addressed to National Labor
Relations Commission, Eighth Division, Cagayan
de Oro City through registered mail at the
Philippine Post Office, Corcuera St., Zamboanga
City;

4.I first went to the office of Atty Czar Ciro

Sedillo at Paseo del Pilar in Sta. Barbara,
Zamboanga City, to personally serve him a
furnished copy of the said Petition;
5.I then went to the office of the NLRC-RAB IX in
San Jose Road, Zamboanga City where I likewise
personally served a furnished copy of the
Petition to the Office of the Honorable
Executive Labor Arbiter;
6.I then placed three(3) copies of the Petition
proper, already signed received by both the
NLRC-RAB IX and Atty Sedillo’s office, inside a
10” X 15” purple-colored envelope. Then I added
the plain white 4”x9” mailing envelope
containing

7.Because of the huge bulk of the documents, I

bundled three(3) copies of the Annexes to the
Petition separately in another purple-colored
envelope. I submitted the two(2) envelopes to
the Post Office on January 20, 2017 and I was
issued two(2) Registry Receipts numbered: RD 667
821 769 ZZ and RD 667 821 755 ZZ;
8.I am executing this Affidavit to attest to the
truth of the foregoing and for whatever legal
purpose this may serve.
