JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

RE: ONE AND THE SAME PERSON

WE, ANTONIETA DELOSO and LESLIE P. FULLIDO, both of legal ages, Filipinos and both residence
of Mandaue City and Liloan, Cebu, respectively, after having been duly sworn in accordance with law, do
hereby depose and say:

1. That we personally know MARSHALIZA CANANE SAN MIGUEL, who is our long time neighbor
and acquaintance at P. Basubas, Tipolo, Mandaue City, Cebu;

2. That we know for a fact that the person by the name of “MARSHALISA CANANE SAN
MIGUEL“ as appearing in her NSO Birth Certificate and the name “MARSHALIZA CANANE
SAN MIGUEL“ as appearing in most of her pertinent personal documents and other
identification cards, refer to one and the same person and that her true and correct name is
MARSHALIZA CANANE SAN MIGUEL;

3. That we have personal knowledge and information of all the aforestated facts because we
are close neighbors and intimate family friends and we frequently visit the said family at
their residence.

4. That we are not in any way related to any of the foregoing person within the fourth civil
degree either by consanguinity or affinity, nor do we have any pecuniary interest in this
matter whatsoever;

5. To remove the confusion as to the two names, we hereby declare that the name
MARSHALISA CANANE SAN MIGUEL and MARSHALIZA CANANE SAN MIGUEL refer to one
and the same person;

6. That the abovementioned discrepancy was caused perhaps through inadvertence, honest
mistake and oversight.

7. That we are executing this affidavit jointly to attest the truthfulness and veracity of the
foregoing facts in establishing the real identity of MARSHALIZA CANANE SAN MIGUEL and
for whatever other legal intents and purposes the same may legally serve;
