MEDICATION RECONCILIATION/REVIEW

Drug Indication Mechanism of Action Adverse reaction

Ibuprofen Mild to moderate non-selective Dizziness, headache,
pain and fever inhibitor of an edema,
enzyme called agranulocytosis,
cyclooxygenase aplastic anemia,
(COX), which thrombocytopenia
is required for the
synthesis of
prostaglandins via
the arachidonic acid
pathway. COX is
needed to convert
arachidonic acid to
prostaglandin H2
(PGH2) in the body.
PGH2 is then
converted to
prostaglandins.
Calcium Carbonate + Hyperacidity Famotidine is an H2- Headache,
Magnesium antagonist Constipation,
Hydroxide + Diarrhea
Famotidine tab Calcium carbonate &
magnesium
hydroxide are
antacids
Sodium ascorbate Vitamins.
Ascorbate quickly
accumulates in
microvascular
endothelial cells,
scavenges reactive
oxygen species, and
acts through
tetrahydrobiopterin
to stimulate nitric
oxide production
by endothelial nitric
oxide synthase.
Sotalol Arrhythmia Antiarrhythmic: Class Headache, weakness,
II (beta blockade) and anxiety, erectile
class III (action dysfunction,
potential hyperglycemia,
prolongation) bronchospasm
properties

Has adrenoceptor-
blocking effect and
markedly prolongs
action potential and
repolarization
Captopril HTN, CHF Angiotensin Dizziness, abdominal
converting enzyme pain, nonproductive
(ACE) inhibitors dilate cough, dyspnea,
arteries and veins by angioedema
competitively
inhibiting the
conversion of
angiotensin I to
angiotensin II (a
potent endogenous
vasoconstrictor) and
by inhibiting
bradykinin
metabolism; these
actions result in
preload and afterload
reductions on the
heart

Hydrochlorothiazide Hypertension, Heart Thiazide diuretic; Dizziness, headache,

failure inhibits sodium pancreatitis,
reabsorption in distal anorexia, epigastric
renal tubules, disease, renal failure,
resulting in increased polyuria,
excretion of water anaphylactic
and of sodium, reactions
potassium, and
hydrogen ions
