TASK 1. PRE-SIMULATION SCENARIO SCRIPT


Instructions: Based on the scenario below, create a script and role play the given situation. In the
presentation, exhibit the appropriate approach on how the nurse/nurse manager will handle the situation.
Using virtual role play simulation, you may record it in any virtual platform or recording media. Video
editing is allowed. (See rubrics for scoring)

Scenario:
Two beginning nurses are working a unit and have recently gone through orientation together. Beginning
Nurse A has just learned that 3 hours earlier Beginning Nurse B had given an incorrect (overdose) of albuterol
to an asthmatic patient with a history of cardiac disease which resulted in the patient becoming tachycardic
and hypotensive. The nurse discovered the error when attempting to remove the albuterol from the medication
cart for the next scheduled dose when beginning nurse B was on her break. There was only one dose left
instead of the 2 doses. She contacts respiratory therapy to inquire if they had given an extra dose; no
medication was administered by RT.

Background information:
The nurse who committed the error is a new graduate, recently off orientation and is scheduled for her 6-
month performance appraisal next week. The nurse who discovered the error is also a new graduate who
completed orientation with the other nurse. The new graduate B who made the error has 4 patients one of
whom was unstable, and she had also been asked to assist another nurse whose patient was crashing.

Roles:
Crestine Sabaulan as the Beginning Nurse A
Airrah Valencia as the Beginning Nurse B
Ranz Kenneth Frando as the Respiratory Therapist
Dianne Kim Francisco as the Nurse Manager

Different Situations:
A. The nurse making the discovery reports the error to the nurse manager. Nurse B tells Nurse A “We
went through orientation together and became friends. I thought you had my back. I never expected
you to rat on me like that. The patient was not harmed.” What do you, Nurse A say? How the Nurse
Manager deal this report?
(STUDENTS ROLE PLAY BEGINNING NURSE A AND BEGINNING NURSE B, Nurse A
BEGINS BY STATING – “Nurse Manager, I discovered a medication error, Nurse B gave 2
albuterol doses to Mr. RM instead of one. I checked with respiratory therapy and they did not give an
in-between dose”)

Si nurse crestine na isang baguhang nurse ay nakadiskubre ng medication error sa kanilang unit na
syang gawa umano ng kaibigan nyang baguhang nurse din na si Airrah. Nangyari ito nung kanyang
tanggalin ang gamot sa medication cart upang maihanda ang susunod na dose nang napansin nyang
kulang na ang gamot kung kaya’t siya ay napaisip at nagtanong sa kanilang Resp. therapist ukol dito.
Matapos nya itong tanungin at makumpirma ang hinala ay naisip nyang ireport ito sa kanilang nurse
manager.

Nurse A: Good morning po nurse manager Kim, may nais lamang po sana akong ireport tungkol sa
aking nadiskubreng medication error na naganap. (normal tone)

Nurse Manager: Good morning Ms. Crestine. Pakisalaysay ang buong pangyayari.

Nurse A: Si nurse Airrah po kasi nagbigay ng 2 dose ng albuterol kay Mr. RM yung pasyente po na
may asthma at history ng cardiac disease, na dapat po ay isa lang. Tinanong ko na rin po si Sir Frando
yung respiratory therapist po kung nagdagdag po ba sila ng dose o nagbigay sila in between dose kasi
kulang po yung gamot sa medication cart, tapos ayun po wala daw po silang idinagdag o binigay na
in between dose sa pasyente. Ihahanda ko po kasi yung gamot kaya tinanggal ko po sa medication
cart, tapos ayan po ang nadiskubre ko. (mahinahon tone)

Nurse Manager: (nag-iisip) thank you nurse Crestine for that information. I will make an
investigation to that and to make it fair I also want to hear the side of nurse Airrah. (authoritative)

Nurse A: Yes po, Nurse manager Kim. Salamat din po.

At tuluyan na ngang nag-imbistiga si nurse manager Kim tungkol sa pangyayaring naganap. Upang
makasigurado sa katotohanan ay ipinatawag nya rin ang nasabing nurse na may kasalanan at
pinakinggan ang kanyang side saka pinag-isipan ang kanyang gagawing hakbang ukol sa insidente.
(auds pagandahin mo nalang yung ganap ng narrate hahahahaa more on sa pagdeal ni nurse manager)

Sa kabilang dako, matapos ipatawag at hingin ang kanyang side tungkol sa insidente, ay agad
naglabas ng saloobin si nurse Airrah kay nurse Crestine.

Nurse B: nurse Crestine, hindi ba sabay tayong nag-orientation at naging magkaibigan na rin tayo
pagtapos non. Pero bakit naman ganon? Akala ko mauunawaan mo ko. Akala ko hindi mo ako
ilalaglag. Hindi ko lubusang inakala na ikaw pa ang magrereport sa akin tungkol sa nagwa ko. Lalo
pa’t wala namang masamang nangyari sa pasyente. (naiinis na nangongonsensya tone)

Hindi na nakasagot pa si nurse Crestine sa mga naging pahayag ni nurse Airrah. Nanatili na lamang
syang kalmado at tahimik upang hindi na lumala pa ang sitwasyon gayong nasa duty sila. Mas
pinagtuunan na lamang nya ng pansin ang kanyang ginagawa.

B. Nurse A who made the discovery goes to Nurse B and tells her of the discovery. The nurse who
committed the error pleads with Nurse A not to report her to the manager since the patient was not
harmed, because her evaluation is due. What do you do?
(ROLE PLAY NURSE A AND NURSE B)

Nang malaman ni nurse Crestine ang pagkakamaling naganap, agad nya ito sinabi kay nurse Airrah
upang makumpirma na din ito mula sa kanya mismo. Hiniling naman nito na huwag na siyang ireport
sapagkat wala namang masamang nangyari sa pasyente at sa kadahilanan din na nalalapit na ang
kanyang evaluation.
Nurse A: nurse Airrah, may sasabihan ako sayo. (unsure tone)

Nurse B: Yes, ano yon nurse Crestine?

Nurse A: nurse Airrah kasi ano, ummm…(nag-iisip kung paano sasabihan) may nadiskubre akong
medication error nung shift mo. Kaya naisip kong ireport kay nurse manager Kim.

Nurse B: Hala, nurse Crestine wag. Huwag mo sanang sabihin ang nagawa ko kasi wala namang
nangyaring masama sa pasyente tsaka malapit na rin evaluation ko. Parang awa mo na.. (takot na
nagmamakaawa tone)

Nurse A: pero hindi ba iyon ang dapat na gawin?

Nurse B: Wala namang nangyaring masama sa pasyente. Tsaka malapit na evaluation ko.. kaya sana
wag mo na ireport. (persuasive tone)

Nurse A: nurse Airrah, mahirap kasi yang hinihingi mo. Nasa iisang unit tayo, kung may pagkakamali
kang nagawa madadamay din ako siyempre. (mahinahon tone)

(insert sound effects) Paging nurse airrah please proceed to room 205.

At tuluyan na ngang natapos ang kanilang pag-uusap nang ipatawag si nurse Airrah. Sa kabilang
dako, patuloy paring iniisip ni nurse Crestine kung anong gagawin. Alam niya kasi na madadamay
din sya kung sakaling pagtakpan nya ang kaibigan para sa evaluation nito. Ngunit, alam niya din na
dapat niya itong sabihin upang hindi na maulit pa ang ganitong pangyayari lalo pa’t alam nyang may
hawak na unstable patient si nurse Airrah at kung magkamali ulit ito, maaaring may mangyari sa
pasyenteng ito.

C. Beginning Nurse A decides not to tell anyone about her discovery. However, the respiratory therapist
is on the unit and asks you “What happened with that albuterol discrepancy?” The nurse manager is
with him and wonders to what he is referencing? What do you do? How will this play out?
(ROLE PLAY RESPIRATORY THERAPIST, THE NURSE MANAGER, AND BEGINNING
NURSE A)

Napagdesisyunan na ni Nurse Crestine na wag sabihin ang insidente. Ngunit bigla silang nagkita-kita
ng respiratory therapist na si Frando kasama ang Nurse manager. Agad na nagtanong si Frando kung
ano na ang nangyari sa aberya sa gamot na albuterol. Mababakas naman sa mukha ng nurse manager
ang pagtataka sa kung ano ang sinasabi ni Frando gayong wala naman syang natatanggap na report
ukol dito.

Nurse A: Magandang hapon po, Sir Frando and nurse manager kim.

Resp. therapist: magandang hapon din sa iyo nurse Crestine. Ano na pala ang nangyari dun sa
problema sa gamot na albuterol? (curious)

Nurse manager: anong problema yan Mr. Frando? Bakit wala akong natatanggap na report about that?
Resp. therapist: Kasi nagtanong sa akin itong si nurse Crestine kanina tungkol sa gamot na albuterol
kung nag-administer daw ba ako in between dose o nagdagdag sa dose kasi daw kulang na yung nasa
medication cart.

Nurse Manager: Paanong nagkulang nurse Crestine?

At wala na ngang nagawa si nurse crestine. Tuluyan na nyang isasalaysay ang mga pangyayari
patungkol sa nagkulang na gamot. Naisip nya na wala naman nang iba pang paraan kung di sabihin
ang katotohanang hiniling itago ng kaibigang nurse sapagkat mas malalagay lang sila sa alanganing
sitwasyon kung di nya ito sasabihin. Maari ding maulit pa ito at di magawan ng sapat na aksyon na
maaaring magdulot ng problema sa pasyente, sa kanilang trabaho at maging sa organisasyon.

Nurse Crestine: nurse manager kim, may isang nurse po kasi ang nakagawa ng medication error po.

Nurse Manager: Medication error? Kung gayon, maaari ko bang imbitahan ka sa aking opisina pati na
rin ang nurse na tinutukoy mo?

Nurse Crestine: Yes, nurse manager kim. Thank you po.

At tuluyan na ngang nag-usap ng pribado sila nurse manager Kim at ang dalawang involved na nurse
sa nasabing insidente.

D. The nurse manager now summons the nurse who committed the error and the nurse discovering the
error together to discuss the situation and initiate the incident report that needs to be filed. The nurse
manager discusses the situation privately with the nurse who commits the error. How should this
conversation proceed? Based on the principles of a just culture what should happen?
(STUDENTS PLAY THE ROLE OF THE NURSE MANAGER AND BEGINNING NURSE A. The
scenario shall present on how to utilize the just culture decision making model)

Ipinatawag na nga ni nurse manager Kim ang 2 upang makausap ng pribado ukol sa nangyaring
pagkakamali at sinabihan din silang magpasa ng incident report ukol dito.

Nurse A and B: Magandang hapon po.

Nurse manager: magandang hapon. Nurse Crestine, maaari mo na bang ilahad kung ano ang
pangayayring tinutukoy nyo ni Mr Frando?

Nurse A: Yes po, Kasi po nung nagchecheck na po ako ng mga gamot sa medication cart para po sa
next na pagbibigay e napansin ko pong nagkulang na yung albuterol para kay Mr. RM. Una ko pong
ginawa ay nagtanong po kay Sir Frando kung nagdagdag po ba sila ng dose baka kasi doble yung
binigay na huli since nagdagdag po sila or sila po ba ay nagbigay ng in between dose? Sa parehong
katanungan po ay hindi ang natanggap kong sagot. Kaya ang tanging naiisip ko nalang po ay baka
nagkamali si nurse airrah at doble nga ang naibigay nya imbes na isa lamang.

Nurse manager: nurse Airrah anong masasabi mo?

Nurse B: Nurse manager, pasensya na po. Inaamin ko pong nagkamali ako ng bigay sapagkat nadoble
ko po ang dapat na isa lang.
Nurse manager: nurse airrah, anong dahilan?

Nurse B: Pasensya na po talaga ma’am. Hindi ko na po nadouble check sa sobrang gutom ko po nung
mga oras na iyon kaya po inaako ko po ang responsibilidad sa pangyayari.

Nurse manager. Nurse airrah, alam mo namang may kaakibat na responsibilidad ang mga ganyang
pagkakamali diba. Kaya sana sa susunod mas maging maingat tayo sa mga ginagawa natin kahit ano
pa ang nararamdaman natin. Kailangan nating magawa ng maayos ang trabaho natin dahil kung hindi,
kawawa ang ating mga pasyente,

Nurse B: opo ma’am.

Nurse manager: Ikaw din nurse Crestine, ang hindi mo agarang pagreport ay nangangahulugang
pinagsawalang bahala mo ang pagkakamali ng iyong co-nurse o di kaya ay pinagtakpan mo siya.
Sana ay nauunawaan mo ang ating polisiya ukol sa medication error kung bakit ito dapat ireport at
hindi ipagsawalang bahala.

Nurse A. opo ma’am, nauunawaan ko po.

Nurse manager: okay, both of you will create an incident report about the matter. We will have our
conference later with the whole staff about how we will resolve this kind of matter so that it will be
prevented or will not happen again. Medication error is very dangerous for our patient’s health
because as a nurse we must always be aware that this profession does not have any room for
committing mistakes. We nurses are working to provide care and not to cause harm in our patient. I
hope this never happen again nurse airrah and nurse crestine.

Nurse A and B: yes po, nurse manager kim.


Nurse manager: I hope you learned from this, nurses. I’ll see the both of you gain in our conference
later. You may leave now.

Nurse A and B: yes po, thank you nurse manager kim.

Nurse B: pasensya na po ulit.
