WAIVER OF RIGHTS

I, RIZA C. CAINGING, of legal age, single, Filipino and a resident of Poblacion

del Norte, Villaba, Leyte, Philippines, after having been duly sworn to in accordance
with law, do hereby depose and state that:

1. I am the SK Chairman of Brgy. Poblacion del Norte, Villaba, Leyte;

2. That on April 2021, I disobey the rules and regulation of the Barangay as SK
Chariman in Brgy. Poblacion del Norte, Villaba, Leyte;

3. That on April 27, 2021 I issued check and encash from the Land Bank of
Ormoc City, Leyte without the permission or exact seminar of all brgy.
officers;

4. I hereby waive my position as SK Chairman of Brgy. Poblacion del Norte,

Villaba, Leyte in favor of EDMON C. NOVAL, all rights and interests that I may
have over any from the abovementioned position due me from SK Chairman,
as the holder thereof;

5. I hereby release and discharge my position as SK Chairman from any and all
liability in connection with the aforementioned waiver and release of the
benefits, monies, rights and interests over the abovementioned SK Chairman;

6. I am executing this Affidavit to attest to the truth of the foregoing and for
whatever legal purpose that it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 6th day of July,
2021, at Villaba, Leyte, Philippines.
