SUBJECT : Spot Report Shooting Incident that Resulted

to death of Mr Dick choyaks y Urdas

DATE : April 25,2010

1. On Nov 26, 2019 at about 3:00 o’clock in the afternoon shooting incident
transpired at Sacred heart College Soutwestern Pob, Calamba, Mis Occ where in victim
is Mr Dick choyaks Y Urdas , 46 years old, married, Dean, and a resident of So. Brgy
Calaran ,Calamba Mis Occ ,was shot by Unknown suspect. As a result thereof, the
victim was standing through the book case at the library. While the victim choosing a
book there was a man came up and have a helmet at the backside of the victim and
suddenly the unknown suspect get his gun and shoted three times in the chest part of
victim and was rushed to Calamba General Hospital Misamis, Occidental for medical
treatment but was pronounced dead on arrival (DOA) by the attending physician
thereat. The Unknown suspect was not arrested and still at large.

2. The motive of incident is undetermined and the case still under

investigation.
