I, FELIX JEFF OUMA of P.O BOX 17680-20100 NAKURU hereby petition this
Honourable Court for a grant of letters of Administration ad litem of the estate of the
above named person who died domiciled in NAKURU COUNTY on the
limited as personal representative for purpose of filing suit and without power of
distribution of the estate and say as follows;-
1) That I intend to substitute the deceased in CIVIL SUIT NO E669/2021 as the
plaintiff and continue to prosecute the matter on behalf of the estate of the
deceased.
2) I present this petition in my capacity as the _SON_____
3) That a grant of administration ad litem do issue limited for the purpose of filing
suit.
4) That we have no powers to distribute an estate under this grant.

AFFIDAVIT IN SUPPORT OF PETITION FOR LETTERS OF

ADMINISTRATION AD LITEM.

I, FELIX JEFF OUMA of Post Office Box Number 17680-20100 NAKURU within
the Republic of Kenya do hereby make oath and state as follows;

1. THAT the full names of the deceased whose estate the proceedings relate is
AGGREY OUMA ONYANGO and I am the Petitioner named in the Petition
filed herewith.

2. THAT the Deceased died …………………. (Attached herewith is a photocopy

of Death Certificate marked ‘FJO-1’ attesting as much.)

3. THAT the Deceased was at the date of his death domiciled in Kenya and his
last known place of residence was at NAKURU COUNTY within the
Republic of Kenya.

4. THAT the deceased died intestate. (Annexed herewith and marked ‘FJO-2’ is
a letter from the chief attesting as much.)

5. THAT I am related to the Deceased in my capacity as his son and I have the
consent of………………...to lodge this Petition. (Annexed herewith and
marked FJO-3 (a), (b)are copies of our National Identity cards attesting as
much.)

6. THAT the Deceased had instituted Civil Suit No. E/E669/2021 on 16th July,
2021 against one JOHN GITAU and the matter is still pending in court.
(Annexed herewith and marked FJO-4 is a copy of the Plaint attesting as
much)

7. THAT I swear this Affidavit in support of the Petition for Letters of

Administration ad litem to enable me to substitute the Deceased as the
 plaintiff in CIVIL SUIT NO. E/669/2021 on behalf of the estate of the
Deceased.

8. THAT what is deponed to hereinabove is true to the best of our knowledge,

save for what is deponed on information and belief, sources whereof have
been disclosed.
