6. Pulmonary edema and congestion are common post-mortem findings.

Discuss the nature of their presence (4 marks).

POST-MORTEM FINDING NATURE OF THEIR PRESENCE

Pulmonary edema  Excess fluid accumulation in the lungs;

resulting to difficulty of breathing (Brazier,
2021).
 Generated by a bacterial enterotoxin
(verotoxin) produced in the small intestine and
disseminated via the bloodstream by inducing
IL-8 (Gelberg, 2017).
 Proteases, in combination with oxidants, can
kill alveolar type 1 cells and perforate the
epithelial-interstitial and interstitial-endothelial
barriers in the lungs (Lucas, et al., 2020)
 Exotoxin Y disrupts the pulmonary barrier by
causing epithelial and endothelial cell death
leading to pulmonary edema (Lucas, et al.
2020)
 Listeriolysin O promotes the establishment of
pulmonary edema by at least two
3

mechanisms: capillary leak and impaired liquid

clearance (Lucas, et al., 2020).
Congestion  Eosinophilic intranuclear inclusion bodies have
been linked to necrosis foci in lungs and liver
inducing pulmonary congestion (Brockmeier,
Halbur, & Thacker, 2002).
 Primary infections are worsened by
opportunistic germs, it leads to more serious
and persistent respiratory disease and
congestion. Porcine reproductive and
respiratory syndrome virus, swine influenza
virus and pseudorabies virus (Brockmeier,
Halbur, & Thacker, 2002).
