Situation: Jamie Perez, a student nurse, paid a visit to Barangay


Tabok. He met the Saromines family in the area. Mrs. Saromines
was there, together with her 3 children.

GOAL/ GENERAL OBJECTIVES:


After 1 week of home visiting, the client as well as the family will
be able to acquire adequate knowledge, appropriate skills and
positive attitude in the promotion and maintenance of health and
prevention of illness in the care of the family.

SPECIFIC OBJECTIVES:
After 30 minutes of student nurse- client interaction, the student
nurse will be able to:
1. Establish rapport with the client and family members
2. Define Home visit and know its importance and benefits
3. List out the health needs of individual and family members
related to the environment
4. Formulate an appropriate health care plan from the identified
health problems.
5. Set contract of the next home visit

NURSING ACTIONS:
1. Establish a positive relationship with the family
• Introduce self to the client as a student nurse of Cebu Doctors’
Univeristy in a polite and respectful manner
•Exemplify the core values of a CDU CN student
• Apply therapeutic communication techniques.
• State the name of each family member

2. Define home visit and know its purpose and importance.


• Define home visiting to client’s level of understanding
• Explain the importance of home visit

Home Visiting is a professional, purposeful interaction with a family


in their home with the goal of promoting, maintaining, or restoring
the family's or members' health.

3. List out the health needs of individual and family members


related to the environment.
• Know the health status of an individual or a family
• Question the family members about health problems they
commonly experience in their household.
• Interview and Make a family care plan that resolve the
identified problems utilize family assessment tools such as the
Initial Data Base (IDB) and the Family Coping Index (FCI) to
identify potential and actual problem
 Anxiety Attacks due to loss of loved one
 Financially Unstable
 Stress and Trauma

The family introduce themselves wherein Mrs. Antoinette

S Saromines a widowed mother and has three children; Mr. Ray


Saromines as the eldest, Ms. Ellaine Saromines as the only girl,
and Mr Morton Saromines as the youngest.

O The family was struggling physically, emotionally and financially


because the breadwinner of the family had an accident.

Anxiety related to actual loss of significant others as evidence

A by anxiety attacks, blood pressure and heart rate elevation, and


situational issues currently in the patient life.

P After 1 week of nursing care, the family will be to express their


feelings and concerns appropriately

I • Assess the family’s psychological and physiologic comfort.


• Encourage the family to verbalize own anxiety and coping
patterns.
• Help the family develop 3 coping mechanisms to help with
individual anxiety attacks.
• Encourage the family to explore possible stressors and
lifestyle changes that they can change in order to help with the
anxiety in their life.

physiologic comfort.
• The family will effectively use 3 coping mechanisms to help
with anxiety attacks.
• The family can take in control of the situation, especially if
anxiety attacks occur.
