Subject: Notice of Expulsion of Mr. ____________

Notice is hereby given that Mr. _________ is officially expelled from our
Fraternity. The reason of the expulsion being that Mr._________ has joined another
Greek Letter Fraternity without undergoing our traditional rites by signing our Quit
Form and the sacred embedding of an “X” mark in his burn which symbolizes our
identity in our Fraternity.

Article 11 –Grounds for Investigation and Sanctions

25. Joining other fraternal organizations without having been formally

discharged from the Beta Phi Sigma by use of Quit Form.

By the powers vested in me by our Constitution and By-Laws, as the National

Council Chairman, effective immediately, Mr. Mark Matuguina from Delta Zeta
Chapter, Barangay Poblacion, Medellin Cebu, is hereby stripped of his membership
in our Fraternity.

It is also ordered that Mr. Mark Matuguina will not be under the
responsibility of the Beta Phi Sigma Fraternity and will be prohibited to use our
Official Seal for whatever purposes. Also, he will be stripped of his right and
privileges under the Seal of the Beta Phi Sigma.
