Nov 10 Carol L. Javier Teacher 1 C: There was an absence of learning output from week
1 to 3 and based on the teachers’ class record found
out that there were no submissions, and it is found out
at home.
R: Student need to submit for the week 1 to 3.
A. Proper communication to their parents as well as
the students due to low scores in their summative
assessment. This should properly be addressed to the
students and parents. the teacher needs to give
additional supplementary activities for the students to
achieve better learning and understanding of the
lesson for week 1 to 3. Also, close monitoring of the
students' progress is needed to be sure that all tasks
are completed and will be accomplished well.

Nov 12 Janice D. Larez Teacher II C: 1. Student 1 assisted by an older sister home


facilitator, but still the monitoring of outputs was not
properly addressed and forgot to submit her output
every Friday. The student was advised to answer all
learning tasks, because she forgot to answer and
resulted in low scores on formative and summative
assessments.
2. Student 2 was not able to follow the direction on the
activities for week 1 to 3 and was not able to answer
the learning task. The teacher offered some references
for the students to answer the learning task specifically
for a performance task which requires critical thinking
skills like creating a paragraph based on the story
read.
3. Student 3- The formative and summative
assessment was very low specifically for the
performance task, the students were not accomplished
by the students. The teachers advised the students
and parents to help accomplish the task. The task that
will not accomplish having a great effect on her
grades. The teacher asked the students what the
hindrances are overcome by the students and offered
references and help the students to increase the level
R: If there is assistance from parents, the possibility of
getting high scores in all activities and student will
accomplished all given tasks.
A: The teacher needs to give additional supplementary
activities for the students to achieve better learning
and understanding of the lesson for week 3. Also,
close monitoring of the students' progress of teachers
and parents is needed to be sure that all tasks are
completed and will be accomplished well. The students
may undergo a reading intervention program to
increase her critical thinking ability.

Nov 16 Princes V. Ramos Teacher I C: The teacher discussed thoroughly the result of
formative and summative assessments to student
and guardian specifically for Week 4. Although all
learning tasks have been submitted to the school
except for week 4, the record showed the low
scores in most of the learning tasks submitted.
Although the guardian is persistent to follow up his
daughter but still the assistance of a guardian was
requested by the teachers and agreed that she will
assist her daughter in answering the learning task.
R: If the student had difficulty in answering the written
and performance works, the teacher needs to give
additional supplementary activities for the students to
achieve better learning and understanding of the
lesson for week 4.
A: Close monitoring of the students' progress is
needed to be sure that all tasks were completed and
will be accomplished well. The teacher also mentioned
that all the questions of students are being entertained
for her to cope up with lessons, and the student was
encouraged to ask questions if some activities or
direction is being misunderstood.
The teacher and parent need to agree for a close
monitoring of the student, since some output has not
yet been submitted to school. There should also be
proper communication to their parents as well as the
students due to low scores in her formative
and summative assessment. This should properly be
addressed to the student and parent that the teacher
will give supplemental activities.

availability, some students with incomplete
answers. Some students do not have name on the
output.
R: If the students are late for submission
coordinate with guardian through FB messenger,
telephone call, or via SMS.
A: Agreed to show the class record and close
monitoring of students in coordination with home
facilitator or guardian. Emphasize the proper
naming of all learning task in the FB messenger
since majority of students have internet
connection via messenger.

A: Communicate with students and parents to guide
them in accomplishing the weekly activities. Remind
the students and parents the importance of
accomplishing all the task, and the teacher monitor

risk in her class. Based on the exchange conversation
between the guardian and the teacher, the teacher
was able to recommend solutions based on the
progress monitoring report as reflected in the class
record. She also discussed the result of formative and
summative tests to parent in absence of the student.
The teachers also ask clarifications on the task given
to the student due to unaccomplished learning
tasks, specifically the performance task. The teacher
highlighted in her class record those tasks that are
accomplished by the students and discussed it with the
parents.
R: Student A Students need close monitoring of
parents in accomplishing weekly task. Students
with no submission of output which found left his
guardian and cannot be located.
If the students cannot be located by the guardian,
need to report to the school head. Emphasize the
importance of close monitoring of parents.
For student B, the guardian informed the teacher that
the student left the house already and there was no
contact with this student. He has not output from week
1 to 6, the student cannot be traced by the guardian.
She informed the teacher that he had family already
and they cannot reach out to the student since he left
the house. There was no update for this student.
The parents and teacher agreed to finish the
task as soon as possible or before December 9, 2020.

A: Close monitoring of students, and other students


who got zero scores were recommended. Also, there
should be an intervention program, or the teacher may
give other supplemental activities for the students who
got low scores in the performance task. Close

with students at risk in her class.

A: The student may undergo a reading program at


home and monitored by the teacher, as reflected in his
unfinished task, the student was not able to
comprehend and speak in English. This is very
important for TVL student to speak, reading directions
for these is important for TVL students. Close
monitoring of these students, and other students who
got zero scores were recommended. Also, there
should be an intervention program for the students
who got low scores in the performance task. They
need more practice in welcoming, seating and taking
orders for the customers to master the learning
competencies for week 1 to 3.
Most of the students in the class record have
an incomplete written and performance task, the follow
up or strict monitoring by the teachers should be
reinforced.
