Subject: Reiteration of Memorandum on Leave of Absence with Additional

Information in Filing Sick Leave of Absence

This is to reiterate the Memorandum No. 21-06-LKI-001 dated June 8, 2021 regarding Leave of
Absence. Again, please read and review the memorandum posted in our group chat (LKI HR &
Admin. Concerns) and bulletin board.

In addition, please be reminded that any employee who will go on sick leave, regardless on the
number of days, is required to furnish the office proof of illness upon filing, such as a doctor’s
certificate/medical certificate/note from the attending physician on the day the employee reports
back to work. Failure to present any proof of illness shall be considered unauthorized and
disapproved sick leave of absence, thus, may be a ground for disciplinary action.
