Affidavit of Consent with Authorization

I, Jake A. Javison, of legal age, Filipino, single, a resident of 868 Tabuc Tubig,
Dumaguete City, Negros Oriental, Philippines, on oath hereby state as follows:

1. I was married to Eyra A. Lazarte and during our married we were blessed with two (2)
children namely Jaila Lazarte Javison, born on July 28, 2004 and Ace Mark Lazarte
Javison, born on January 13, 2006;

2. Our children have been under the care and custody of Eyra’s mother Eveilla P. Amosin
with our consent;

3. For the best interest of our children, I hereby give my consent in favor of Eyra A. Lazarte
to take full custody of our children and give her full authority to decide on matters which
redounds to the best interest of our children;

4. I further give my consent to allow our children to live with their mother Eyra A. Lazarte
in Dubai and to pursue their education therein without further objection or opposition;

5. I am fully aware that this will be for the best interest of our children and I hereby give
this consent freely and voluntarily for whatever legal purpose this may serve.
