AFFIDAVIT OF AUTHORIZATION

I, JOEL T. ARAULA, Filipino, of legal age, married, a resident of Bacong, Negros

Oriental, on oath hereby state that:

1. I am the owner of a one (1) unit Mitsubishi Adventure being the vendee thereof by virtue
of a Deed of Absolute Sale identified as Doc. No. 183; Page No. 37; Book No. XXXVI; Series of
2019 in the Notarial Register of Atty. Randolph P. Baquiano, a copy of which is hereto attached;

2. The said vehicle is more particularly described as follows:

A copy of the Certificate of Registration is hereto attached.

3. The said vehicle is under ALARM by the LAND TRANSPORTATION OFFICE or LTO
and the same cannot be transferred or registered;

4. I am authorizing MARVIN H. SANICO, a resident of Brgy. San Pablo, Ormoc, Leyte to

secure a Certification from the LTO in Ormoc City regarding the cause of the alarm, when the
alarm was posted, who posted the alarm and other relevant detail;

5. The said Certification will be used as part of the evidence which I may file against the
proper parties.
