Inspection Findings: (Vessel was inspected during the Turnaround)

UT thickness readings were performed throughout.  A localized area in the shell near N-


1 was corroded below the renewal thickness.  Weld buildup was performed to a 60 sq. in or
390 cm2 area to bring this area back up to original thickness.

Nozzle N-4 was corroded and replaced in kind.  The remaining wall thickness was
only 0.381 cm / 0.15 in.  Nozzles N-2 and N-3 were also corroded but are estimated to
make the next turnaround.

Details of the leak: 

INSPxx-PL-001 had a leak on the backside of the first elbow downstream of INSPxx-ES-


170A. The leak was a result of Corrosion Under Insulation (CUI). On the elbow was a
localized corroded area about 100 x 150 mm (3,9 x 5,9 in). The actual leak was a pit about
0.02 mm (less than 0.25 in) diameter. The insulation on this elbow had been damaged. A
Temporary Leak Repair was installed.

The RAM criticality for this equipment is Medium.  This pipe was part of the RBI process
and the mechanism was expected.

The leak was clamped. The elbow should be replaced by 1/Feb/2022. The total impact of
this leak was USD 110,000 (USD 10,000 to perform the repairs, and USD 100,000 process
loss).
