Threats:
1. Fall injury
2. Fatality/ Damage to the asset.
3. Contact with concrete mix
4. Environmental damages
5. Personal injury/ temporary disability.
6. Occupational Illness
7. Personal injury due to ‘fall into’ trenches.

Corrective actions:
1. Barricade the excavated pit. Put up warning
signs, lights etc. applicable.
2. Follow PTW procedures, if need to work in
Confined space.
3. Use appropriate PPE while working with/
handling concrete mix.
4. Secure the shuttering materials, de-nailed .
5. Use the safe Hand tools. Change the damaged
hand tools.
6. Do not wash the concrete dumper and let the
wash spill on the ground. Scoop up the dropped
concrete and dispose off to waste yard.
7. Barricade the open trenches. Do not work/walk
close to the trench edge. Follow safety procedure
for working in trenches more than 1.2m deep

Emergency Response:
1. First aid to be rendered to the injured,
immediately
2. Inform PM/ HSE/ CM/ Admin for necessary
supports.
3. Contain the spill and initiate spill contingency
procedures.
