SUBJECT : MODIFIED ENHANCED COMMUNITY QUARANTINE (MECQ) PROTOCOLS BASED ON


IATF OMNIBUS GUIDELINES

I. References:
a. Omnibus Guidelines on the Implementation of Community Quarantine in the
Phlippines

II. This portains to the rules/guidelines aimed to inform and guide people on the
health safety protocols that must still be observed as some people may
misinterpret the GCQ and MGCQ implementation to mean that they would already
enjoy freedom of travel and movement like the way they do before COVID-19
pandemic.

III. In this regard, following are the additional guidelines on the movement of people
in GCQ and MGCQ areas:
a. Governmentl office shall be fully operational with a skeloton workforce on-site.
b. Gathering outside the residences shall be prohibited. Religious gathering shall be
allowed up to the (10%) of the venue capacity, iin accordance the LGU where the
gathering may take.
c. Movement of cargo/delivery vehicles shall be unhamperd.
d. In accordance to the Metro Manila Counsil, Unified Curfew Hours is enforced from
8:00 PM to 5:00 AM, except essential workers, cargo vehicles, and public
transportation.
e. Persons below 18 years old and above 65 years old, with
immunodefiency or comorbidity, and pregnant women must stay
at home.
f. Need proper hygene every day.

INCIDENT REPORT

SUBJECT : INVESTIGATION REPORT ON VIOLATION OF RA 10883 “New Anti- Carnapping


Act of 2016”

DATE : APRIL 01, 2021


1. Reference: Memorandum from Officer-In-Charge, Camp Saturnino dumalo
Bayombong Police Station dated April 01, 2021 with subject same as above;

2. That on April 01, 2021 (1) MARK ANDRE G SILVERIO, 21 years old, single, a resident of
brgy. Quezon Nueva Vizcaya, he reported the motornapping incident that transpired
about 11:00 in the evening of the same date at 24/7 inn Solano-Bayombong Boundary
3700, Bayombong, Nueva Vizcaya wherein one (1) Honda XRM, color grey, bearing
with plate no. 7974 with engine #XRM18E008050, chasis #XRM18008045. Stole and
drive away by unidentified suspect because the suspect is wearing a mask.

3. The victim parked said motorcycle in front of 24/7 inn Solano-Bayombong Boundary
3700, Bayombong, Nueva Vizcaya, then went inside the hotel and resort. Afterward,
he talking to his friends, and heard a noisy type of motorcycles stopped. Thereafter,
when he was about to transfer said motorcycle he found out that was already missing
the victim immediately searched for it and possible who witness the incident and
suddenly he saw the Police patrol and he report what happen. At about 3:00 in the
afternoon April 02, 2021, BPAT personnel of Brgy. Bonfal West Bayombong inform
Camp Saturnino Dumlao Police Station one motorcycle thereat and positively
identified by the victim.
4. Follow-up investigation is still in effect for the possible identification and
apprehension of the suspects. Investigator-On-Case is PO2 Francis P Pascual with CP #
09754902665.

FINAL INVESTIGATION REPORT

WHERE : 24/7 inn Solano-Bayombong Boundary 3700, Bayombong, Nueva Vizcaya

WHEN : April 1, 2021, at about 11:00 in the evening

SUBJECT : Initial Investigation on New- Anti Carnapping act of 2016

1. To possibility of identify the suspects that will led to their possible prosecution.

III. FACTS/INVESTIGATION/FINDINGS
At about 11:00 in the evening of April 01, 2021, one of the team of Camp Saturnino
Dumlao Police Station is under patrol, suddenly there is an incident of carnapping was
happened in 24/7 inn Hotel and Resort Solano-Bayombong Boundary 3700, Bayombong,
Nueva Vizcaya.

Upon the investigation, the victim go to the 24/7 inn Hotel and Resort Solano-
Bayombong Boundary 3700, Bayombong, Nueva Vizcaya at 10:40 in the evening, he park
his motor cycle in front in the parking lot of 24/7 inn after that he go inside and he talking
to his friends and heard a noisy type of motorcycles, after he heard the noisy of motor
cycle he was about to transfer his motor cycle near to the hotel rooms but he found out
that was already missing, the victim immediately searched for it and possible witness the
incident. The Victim saw the police patrol about 11:00 in the evening and he report what
happen. The witness is one of the staff of the 24/7 inn and the CCTV of the hotel and
resort, the suspect is unidentified because according to the witness and the review of
CCTV the suspect is wearing facemask, cap, black t-shirt and, grey short.

At about 3:00 in the afternoon of April 01, 2021, BPAT personnel of Brgy. Bonfal
West, Bayombong, Nueva Vizcaya inform the Camp S0aturnino Dumlao Police Station,
one motor cycle was found and the victim positively identified. Under the investigation
the suspect identified MR. Piolo S Marcos. According to the witnesses in the brgy. Bonfal
West, Mr. Marcos seen at 11:30 in the evening riding a motor cycle, he wearing cup,
facemask black t-shirt and short the witnesses is very sure in their statement, because the
way he ware, he really recognized. Mr. Marcos caught in his rented apartment.

Camp Saturnino Dumalo, Bayombong, Police Station found out that Mr. Piolo S
Marcos is have a lot of records like carnapping and more. In this case, the witness of one
of the staff of 24/7 inn, the CCTV, the witness of the brgy. Bonfal West and the time of the
carnapping incident and the time he saw the suspect in brgy. Bonfal west, the retrieved
information is identical match.

IV. RECOMMENDATION/DISPOSITION
In view of the foregoing, the victim filled a case to the suspect of RA 10883 “Anti-
Carnapping Act of 2006”
