• Current Year Community Tax
Certificate (Cedula)
• Sketch of business location
• Certificate of any Tax Exemption,
if applicable (Certified True Copy)
• Tax Declaration or any Proof of
Ownership (if owned); or
• Certification from the registered owner
for free usage of his/her property
• For foreign nationals – Authority to Engage Business in the
Philippines, photocopies of VISA and Passport

• If represented, Special Power of Attorney with CLEAR


photocopy of ID of the person authorized to transact &
sign and a CLEAR photocopy of ID of the
taxpayer/applicant

• Board Resolution/Secretary Certificate authorizing person


to transact & sign and CLEAR photocopy of ID of
signatory and authorized person
