AFFIDAVIT

I, RAZEL ALEJADO TUBALLA, of legal age, Filipino, and a

resident of Bonifacio St. District I, Dauin, Negros Oriental, Negros Oriental,
after having been duly sworn, hereby state and declare: that

1. That I am the incumbent bookkeeper of the Liga ng mga Barangay

holding office at the Municipal Hall of Dauin, Negros Oriental;

2. Sometime in May, 2016, Barangay Captain Emerenciana B. Bantoto

went to our office and requested to me if she can look at the checks
issued our barangay from October 2015 to December 2015. I showed
her the checks and there she discovered that her signatures in those ten
(10) checks were forged. I suggested to Barangay Captain to also look
into the checks issued from January 2016 to April 2016, and there she
also discovered that her signatures in those eight (8) checks were also
forged.
