(Correction of Father’s Name)

I, Shri. Jhalak Bahadur Thapa, son of Shri. Hastdal Thapa, resident of 3 NH BN, Assam
Rifles, District- Kohima, Nagaland, do hereby solemnly affirm and declare on oath as follows:
1. That I am a bonafide citizen of India and a resident of the above mentioned address..
2. That I am presently serving as Havildar/GD at 3 NH BN, Assam Rifles, Kohima, Nagaland.
3. That this affidavit is made to declare that in my Service Book my Father’s name has been entered
and recorded as Shyam Bahadur Chhetri instead Hastdal Thapa.
4. That the above mentioned name Shyam Bahadur Chhetri and Hastdal Thapa it refers to one
and the same person i.e. my father and it is indistinguishable and synonymous.
5. That this affidavit shall stand as a piece of evidence for any rectification/ correction if any
complications that may arise with regard to correction of my Father’s name in my Service Book
and records.
6. That this affidavit is signed and sworn before the competent authority declaring that my Father’s
correct official name is ‘Hastdal Thapa’ and henceforth it shall be used for all my Service
records, official records, future references and future correspondence.
That the above statements made in Para 1 – 6 are true to the best of my knowledge and
nothing material has been concealed herein and I signed this affidavit before the Competent
Authority on 22nd May, 2017 at Kohima.

BN, Assam Rifles, District- Kohima, Nagaland, do hereby solemnly affirm and declare on oath as
follows:
1. That I am a bonafide citizen of India and a resident of the above mentioned address..
2. That I am presently serving as Havildar/GD at 3 NH BN, Assam Rifles, Kohima, Nagaland.
3. That this affidavit is made to declare that in my Service Book my Father’s name has been entered
and recorded as Gom Bahadur Gurung instead Gom Bahadur Dura Gurung and my Father’s
name has been entered and recorded as Bir Subba Gurung instead of Bir Subba Dura Gurung.
4. That whereas the entry of Dura in my Name connotes to my Caste.
5. That the above mentioned name Gom Bahadur Gurung and Gom Bahadur Dura Gurung it
refers to one and the same person i.e. me and it is indistinguishable and synonymous.
6. That the above mentioned name Bir Subba Gurung and Bir Subba Dura Gurung it refers to
one and the same person i.e. my Father and it is indistinguishable and synonymous.
7. That this affidavit shall stand as a piece of evidence for any rectification/ correction if any
complications that may arise with regard to correction of my name and Father’s name in my
Service Book and records.
8. That this affidavit is signed and sworn before the competent authority declaring that my correct
official name is ‘Gom Bahadur Dura Gurung’ and my father’s correct official name is
‘Bir Subba Dura Gurung’ and henceforth it shall be used for all my Service records, official
records, future references and future correspondence.
