AFFIDAVIT OF MOTHER’S MINORITY

I, ROSELYN CAMPOS MALABAR, Filipino, of legal age, and a
resident of Purok 3A, Poblacion, Cortes, Surigao del Sur, after having been
duly sworn to in accordance with law, depose and state:

1. That I am the mother of REAN CAMPOS who was born on

November 16, 2002 at Poblacion, Cortes, Surigao del Sur;

2. That at the time I conceived REAN, I was still a minor and

was not legally married to her father, SOPRIANO R. MALABAR, JR.;

3. That she was registered after my maiden name, CAMPOS;

4. That I was legally married one year after her birth;

5. That it is my desire for her to be legitimized as REAN

CAMPOS MALABAR;

6. That I am executing this Affidavit of Discrepancy to attest to

the truth of the foregoing facts and for whatever legal purposes this may
serve.
