Dear Sir,
Letter of Introduction and Request for Collaboration with Caritas Nigeria on “CARES ACT Project” To Increase
COVID-19 Vaccination Uptake in FCT

Catholic Caritas Foundation of Nigeria (CCFN), also known as Caritas Nigeria is Non-for-Profit Humanitarian Relief and Development
Agency, established by the Catholic Bishops Conference of Nigeria (CBCN) as a specialized vehicle to coordinate the social
development interventions of the Catholic Church in Nigeria. The vision of CCFN is “A harmonious environment where everyone
enjoys fullness of life”.

CCFN works to fight hunger, disease, ignorance and respond to emergencies and humanitarian crises by mobilizing resources from
within and outside the Church to meet the basic needs of affected persons regardless of gender, ethnicity, religion, or nationality. CCFN
carries out this mandate in collaboration with the Provincial and Diocesan JDP/Caritas and other local and international humanitarian
agencies.

Caritas Nigeria, has over 10 years’ experience implementing HIV/AIDS and other health projects with the United States Government
funding, Caritas Nigeria is currently one of U.S. CDC Nigeria’s Implementing Partner. We currently support over 140,000 persons
living with HIV in Delta, Enugu, and Imo States, through a robust partnership with the respective State Governments. Recently, U.S.
CDC granted us additional funding to implement the CARES ACT Project, a COVID-19 contact tracing and risk mitigation project in
six States of Lagos, Kaduna, Enugu, Gombe, Plateau and Rivers, including FCT, with the following strategic objectives:

1. To leverage existing faith-based platforms and relationships to strengthen identification and tracking of contacts of COVID-19
cases.
2. To link identified contacts of COVID-19 cases to appropriate structures for risk categorization and management
3. To scale up approaches to community-based risk reduction efforts for COVID-19.
4. To strengthen early notification systems for contacts of COVID-19 cases.

As a strategic and major faith-based organization, we therefore seek to collaborate with you in the Federal Capital Territory (FCT) of
Abuja, where we are presently working with FCT Epidemiologist in contact tracing of persons exposed to confirmed cases, to build
COVID-19 Vaccine confidence which will ultimately result in increase in vaccine uptake in FCT.
