Nursing Plan of Care with vSimulation

vSImulation Client: G.P. 90yr African American male who is diagnosed with minor neurocognitive disorder, Alzheimer’s type 6 months ago and
was prescribed galantamine. He does have hypertension. He is a widow, his wife passed away 3 yrs ago and recently lost his 13 yr. Golden
retriever. He daughter requested this visit because she is concerned her father is becoming more forgetful and appears to be losing weight. CAM
was initially done but the home health agency that was negative for delirium, but some impairment in executive functioning was evident. Mini-
cog was done during the home visit. Pt was independent before his dog died, since then patient does not partake in activities that he would
normally do such as walks. He does occasionally forget to take his medications and forgets to eat.
