AFFIDAVIT OF DISCREPANCY 

I, LUCILA BAUTISTA BECINA, of legal age, Filipino, widow of RESTITUTO PIEDAD BECINA,
and a resident of Brgy. Kabulusan, Magallanes, Cavite, Philippines, after having been duly sworn
to an oath in accordance with law, do hereby depose and state:

1. That I am the widow of RESTITUTO PIEDAD BECINA who died on 14 November 2016 at
MV Santiago Medical Center, Trece Martires City, Cavite as a corroboration of this facts
is hereto attached the faithful machine reproduction of his Certificate of Death as Annex
“A”;

2. That we contracted a marriage to RESTITUTO PIEDAD BECINA on 9 April 2000 at

Nuestra Señora de Guia Parish Poblacion, Magallanes, Cavite as a corroboration of this
facts is hereto attached the faithful machine reproduction of his Certificate of Death as
Annex “B”;

3. That a discrepancy occurs on his Certificate of Death particularly on his Civil Status;

4. That on his Certificate of Death his Civil Status recorded as SINGLE it should be
MARRIED;

5. That his Civil Status is MARRIED by virtue of our Marriage;

6. That I am executing this Affidavit to attest to the truth of the foregoing facts and to use
the same for whatever legal purpose it may serve
