AFFIDAVIT OF DEFECTIVE SIM CARD

I, DAN HENRY R. CRISTICA, of legal age, Filipino and resident of

A. Sanchez Street, Tawiran, Obando, Bulacan, under oath, depose and
say that –

1. That I am a subscriber of Globe Telecom with Mobile SIM Card No.

0956-968-4649.

2. That said Mobile SIM Card No. 0956-968-4649 is duly registered

with GCASH.

3. That when I replace my Cellphone Unit and transfer Mobile SIM

Card No. 0956-968-4649 with my new phone, the same SIM CARD
cannot be used due to “NO SIGNAL”.

4. I tried to switch off and on my phone and remove my SIM Card

then inserted again but the same problem still occurs – “NO
SIGNAL”.

5. I am executing this affidavit to attest the truth of the foregoing and

for whatever legal purposes this may serve more specifically to
request from GLOBE for the replacement of my GLOBE Mobile SIM
Card No. 0956-968-4649.
