Re: Jake Cruz, 22 years old

Dear Team Leader,

I am writing to refer Mr. Jake Cruz, who was diagnosed with major depression and deliberate self-
poisoning. Your management and support are highly requested as he transitions back to the community
today.

Mr. Cruz initially displayed episode of the abovementioned conditions a year ago after his divorce.
Subsequently, on April 5, he presented with the same condition and was commenced on mirtazipine,
30mg and cognitive behavioral therapy. At present, he is showing a remarkable progress, even though
with low mood, there is low risk of self harm, provided he is supported and monitored.

Socially, Mr Cruz is from the Philippines and has limited English skills. Unemployed since last year, he
has been living with his mother who prohibited him from taking antipsychotic medications due to
superstitious beliefs. Hence, education regarding its importance was provided.

It would be greatly appreciated if you could monitor Mr. Cruz closely for any suicidal ideation. observe
his adherence and possible reaction to medication. Kindly ensure that benzodiazepines are not prescribed
as this was used for self-poisoning. Also, continue his therapy with a Filipino clinician or use an
interpreter. Lastly, encourage him to exercise and to look for employment.
