PANGASINAN HELD LAST JUNE 29, 2018 AT 1:30 IN THE AFTERNOON.

PRESENT: PTA OFFICERS AND MEMBERS, FACULTY AND STAFF

Resolution No. 2, s. 2018

RESOLUTION REQUESTING HON. AMADO I. ESPINO III, GOVERNOR, PROVINCE OF

PANGASINAN TO DONATE 100 MONOBLOCK CHAIRS AT PIAZ NATIONAL HIGH SCHOOL.

WHEREAS, the parents identified in a meeting the need of the above mentioned 100
monoblock chairs;

WHEREAS, in response to the pressing need of the school, there is a need of 100
monoblock chairs for classroom and assembly use;

WHEREAS, the school and the PTA cannot proceed with the realization of this project
because of scarce resources;

NOW, THEREFORE, on motion of Jhodilyn Rabago, President- PTA, duly seconded by all
the members, be it resolved as it is hereby resolved to earnestly request the office of the HON.
AMADO I. ESPINO III, GOVERNOR, PROVINCE OF PANGASINAN for the donation of the said
100 monoblock chairs.

BE it resolved further as it is hereby resolved that may this request be given due course
and approval to furnish copies hereof to the office of the HON. AMADO I. ESPINO III,
GOVERNOR, PROVINCE OF PANGASINAN for his information, guidance and appropriate
action.
