ASSOCIATION (PTA), FACULTY and STAFF OF PIAZ NATIONAL HIGH SCHOOL, PIAZ,
VILLASIS, PANGASINAN HELD LAST JUNE 29, 2018 AT 1:30 IN THE AFTERNOON.

PRESENT: PTA OFFICERS AND MEMBERS, FACULTY AND STAFF

RESOLUTION REQUESTING HON. CHINKY PEREZ-TABABA, 5TH DISTRICT BOARD MEMBER,

PROVINCE OF PANGASINAN TO ALLOCATE FUNDS FOR THE CONSTRUCTION OF FACULTY
COMFORT ROOM AT PIAZ NATIONAL HIGH SCHOOL.

WHEREAS, the parents identified in a meeting the need of the above mentioned
construction of faculty comfort room;

WHEREAS, in response to the pressing need of the school, there is a need to

construct faculty comfort room because it is already ruined by termites and of old age;

WHEREAS, the school and the PTA cannot proceed with the realization of this project
because of scarce resources;

NOW, THEREFORE, on motion of Jhodilyn Rabago, President- PTA, duly seconded by

all the members, be it resolved as it is hereby resolved to earnestly request the office of the
HON. CHINKY PEREZ-TABABA, 5TH DISTRICT BOARD MEMBER, PROVINCE OF PANGASINAN
for the allocation of funds of the said faculty comfort room.

BE it resolved further as it is hereby resolved that may this request be given due
course and approval to furnish copies hereof to the office of the HON. CHINKY PEREZ-
TABABA, 5TH DISTRICT BOARD MEMBER, PROVINCE OF PANGASINAN for her information,
guidance and appropriate action.
