                                                 Application Letter of Claim.

I, Mr./Ms ………………………………………………a Filipino Citizen wish to apply for the

claim of the Fund, Four Million One Hundred & Fifty Thousand Euro (€4,150,000.00) deposits
that belong to my cousin late Ma. Jerome who is now dead.

To support my application, I have attached a copy of my personal Valid ID card for proper
identification in reference to the fixed deposit account number (0117547643).

I will not be able to come to Mexico in person to submit my application of claim for some
personal reason but i will be willing to tender my bank account details where the money will be
transferred once my application is approved.

I learned that the deposit will be lost to the treasury department if nothing is done to claim the
deposit within the next 30 days; hence I submit my application for its claim as a Filipino Citizen,
as Cousin of late Ma. Jerome, the fund will be used for missionary and humanitarian project in
the Philippines.
