I, FELIZA L. EFONDO, of legal age, married, Filipino and a resident of B95 L8 South
Fairway Homes, Brgy. Landayan, City of San Pedro, Laguna, after having been duly sworn to
according to law hereby depose and state that:

1. That I am the mother of David L. Efondo, College student of PUP Sta.Mesa ;

2. That I am currently unemployed, as in fact I am merely a housewife;

3. Thus, since I am not gainfully employed, I have no source of income whatsoever;

4. That I am executing this affidavit for the purpose of attesting to the foregoing fact and for
Scholarship Application and for other legal purposes which this affidavit may serve
