2. That I am engaged in the practice of accounting and tax-related activities, where I, by

virtue of my profession, duly represent my clients with the Bureau of Internal Revenue
and other government agencies;

3. That I have never been charged or convicted by final judgment of offense involving
moral turpitude or found guilty of any acts or omissions penalized under the tax code or
found guilty aiding, abetting or causing the commission of any said offense by another;

4. That pursuant to Section 235 of National Internal Revenue Code of 1997, as amended, I
hereby undertake to preserve all working papers, including but not limited to books of
accounts, subsidiary books and other accounting records of my clients within the period
prescribed therein;

5. I hereby further undertake to present said documents and make these available to the
Bureau of Internal Revenue when required to do so;

6. That I am executing this affidavit as a requirement in my application for accreditation

with the BIR..
