Any Equipment with "NO" mark shall be separated and discrepency rectified immediately, A QA/QC re-check shall be perfomed accordingly
LEGEND:
1.1 Equipment is clean and dry with ID number
1.2 Equipment components complete ang tightened accordingly
1.3 F.E/SCBA Latest Hydrostatic Test Date valid
Pressure guage reading (pressurized type) or indicator (cartridge-operated) in the operable range or position and agent fullness determined by weighing
Fire Extinguisher
1.4 (portable) or agent level checked (wheeled units)
SCBA/BA Cylinder Cylinder pressure guage indicators is "FULL"
Fire Extinguisher Maintenance and Operating instructions on nameplate legible
1.5 SCBA/BA Cylinder Labels available and legible
Fire Hose Client color code (if any) painted accordingly
Fire Extinguisher Paint condition acceptable (No sign of stripping)
1.6 SCBA/BA Cylinder Check to ensure high pressure hose coupling is hand tightened to the cyinder valve outlet
Fire Hose Hose rolled with male end coupling inside coil and thread covered with rubber material
1.7 SCBA/BA Cylinder Check that the breathing regulator purge valve (red-knob) is closed. (full clockwise and pointer on know upward). Press and release Air saver switch
Fire Extinguisher Inspection tag or sticker affixed and marked with inspection/service record
1.8 SCBA/BA Cylinder Verify and attach Complete Functionality Test and repair sheet on QA checklist
Fire Hose Hose record card filled-up with B.I.S (Back in Service) remark
