03-01-01-S&L Title WORKING ON BOPS AT HEIGHTS 01/09/99
• Not using safety belts.
• Use safety belts or safety harness whenever working at more than 2
meters height.
03-02-01-S&L Title WORKING ON HYDRIL,CONNECTION 01/09/99
• No safety belt. No proper footholds.
• Wear a safety belt or safety harness when working at heights, properly
secured to a fixed point ABOVE. Ensure that there are adequate
footholds.
03-03-01-S&L Title WORKING IN THE CELLAR 01/09/99
• No safety belt. Part of cellar grating is missing.
• Use a safety belt. Ensure gratings are in place and secure.
03-04-01-S&L Title WORKING ON THE BELL NIPPLE 01/09/99
• Employee is working at height without a safety belt, and without a good
foothold.
• Wear a safety belt or safety harness when working at heights, properly
secured to a fixed point ABOVE. Ensure that there is an adequate
foothold.
03-05-01-S&L Title CLIMBING ON THE BOP 01/09/99
• Employee is working at height without a safety belt.
• Wear a safety belt or safety harness when working at heights.
03-06-01-S&L Title CONNECTING SAFETY CABLES TO SHOCK HOSE 01/09/99
• Too much slack in the safety cable. It may not hold if the shock hose
breaks.
• Make sure there in minimum slack in safety cables.
03-07-01-NDS Title USING THE FALL ARRESTOR 01/09/99
• Safety harness being used, but lanyard is fastened BELOW the worker.
• In this case the Fall Arrestor is safer than using a lanyard.
03-08-01-NDS Title KEEP CELLAR GRATINGS IN PLACE 01/09/99
• Part of cellar grating removed.
• Cellar grating in place giving correct protection in cellar area.

• Store safety gear properly. It is there to save you from injury.
