 B. Mechanism of action:

Co- trimoxazole is a synthetic antibacterial combination product


(Sulfamethoxazole/trimethoprim). It works by inhibiting two consecutive steps in the
biosynthesis of nucleic acids and proteins essential to many bacteria.

Classifications:

General Classification: Antibiotics

Specific Classification: Sulfonamides, folic acid inhibitor, combos

C. Dosage, Route & Frequency: 1-2 DS tablets PO q12-24hr for 14 days

D. Desired effects: This drug is given to treat and prevents infections that are proven or strongly
suspected to be caused by susceptible bacteria.

Independent

1. Obtain client’s history of hypersensitivity reactions to sulfonamides

Rationale:
The drug is contraindicated in those with a hypersensitivity to sulfonamides, it can cause fatal
anaphylactic reactions.

2. Monitor renal function including BUN and creatinine values, and intake and output.

Rationale:
The drug can cause hematological toxic effects, which are increased with kidney disease

3. Monitor baseline and follow-up urinalysis, CBC, platelet count, BUN and creatinine
clearance with long-term therapy.

Rationale: To evaluate effectiveness of drug therapy

Dependent

4. Administer on an empty stomach, one hour before or two hours after meals.
Rationale:
To reduce nausea and vomiting as a side effect of the drug

5. Advise patient to drink plenty of fluids 2.5-3 liters of fluid daily while taking the
medication

Rationale:
To lower the unlikely risk of kidney stones forming, unless physician advises otherwise.

6. Monitor compliance with treatment regimen.

Rationale: Stopping the medication too early may allow bacteria to continue to grow, which may
result in a relapse of the infection and develop resistance to antibiotic.
