Lifting Plan and Check list

Step 1: All persons must visit the actual site and “walk the job” before starting any preparations.
Step 2: Decide what lifting equipment is needed and gather together slings shackles, tag lines,
safety harness & a High Visibility jacket for banksman.
Step 3: Organize the People and equipment and plan the lift using the check list below.

2x webbing slings 24t SWL 14m length , Crane 200t and 120t, crane 50t for man basket
Responsible persons & their key duties for lifting operations: (use tick boxes provided)
Site supervisor  Responsible for all site activities including safe lifting operations.
 Ensures weather conditions are acceptable for lifting operations.
 Ensures a valid permit to work is in place which includes lifting operations.
 Ensures a toolbox talk is done before lifting operations.
 Ensures all lifting equipment and accessories have valid inspection and current certification.
 Ensures his Crane Op, Banksman and load handler are competent and trained.
 Ensure a valid lifting plan is in place for the lift.
Banksman  Wearing Hi –Vis vest he is the sole person controlling and supervising the lifting operation.
 Must ensure all persons not involved in the lift are excluded from the area.
 Must stand back and retain sight of crane op and load handlers.
 Must not touch the load or lifting equipment during the operation.
 Assists the load handlers with preparation and checking of load and lifting accessories before assuming
duties of banksman.
 Checks and ensures the load are free of potential dropped objects.
 Ensure the loads are rigged according to lift plan and rigging plan.
Crane Op  Must be qualified for the crane used and have all required crane inspections & certifications operator
manuals and load charts for the crane.
 Knows weight of all loads, assess the load stability & center of gravity and load path safety.
 Safe operation of the crane using a normal maximum of 75% capacity in configuration required.
 Ensures crane pre-us dropped object and pre start crane check is complete and all safety devices, and safe
load indicators are functioning.
 Check ground conditions for weight bearing capacity, and deploys outrigger pads of 1.44 sq. meter surface
area or bigger as required in the drawing.
 Checks wind speed is less than 9.8 m/s.
 Crane outriggers are lowered, crane is “off rubber “, and outriggers are fully extended and set on the
spreader pads specified in lift drawing.
 Ensure crane is set up & configured correctly for the lifts.
 Uses agreed method of communication with banksman, and follows his directions.
Rigger- load  Prepares the loads for lifting according to the plan and rig up drawings
handlers  Prepares the lifting accessories. Verifies that SWL and WLL are suitable for the load.
 Attaches / detaches the load from the crane on instruction from banksman.
 Use taglines to control the load from a safe distance.
 Remains visible to the banksman during the lift.
Adverse Weather restriction: Lifting will not take place in poor light, poor visibility, sandstorms, heavy rain, or thunder and lightning.
Cranes will not be operated when wind speed exceeds 9.8 meters a second (22 mph /35kph / 19 knots) unless manufacturer allows
operation in greater wind speed.
SIMOPS: No other operation will take place in the lifting area during lifting operations.
PPE: Banksman to wear Hi Vis Vest, all others to comply with standard PPE standards.

Method Statement
Lifting Method statement (Step by Step)
Steps 1-9 are compulsory
1 Rig manager to delegate preparation & check of lift area: free from unnecessary equipment, obstacles and
debris.
2 Crane Op to verify pre start and dropped object inspection of crane has been completed
3 HSE Officer to verify valid certification of crane Operator, crane and lifting accessories -slings shackles
including color coding and that all lifting accessories have suitable SWL / WLL
4 Rig manager to hold Pre job safety meeting/TBT, assign duties for lifting ops.
5 Rig manager to appoint duty banksman (to wear Hi-vis jacket) and load handlers, agree communications
method between Crane operator and Banksman e.g., hand signals. Verify hand signals including stop and
emergency hold.
Agreed communications will be radios, hand signals
6 Banksman to clear area of all person not involved in the lifting: sign and barrier area as required, maintain
an escape route at all times.
7 Crane Op to Spot crane as per lift drawing and planned lift radius, deploy outriggers fully on spreader
blocks, set crane “off rubber”.
8 Crane Operator to carry out configuration safety and computer checks, implementing any specific
operational requirements as per lift drawings.
9 Check wind speed is not exceeding 9.8 meters per second (22 mph /35kph / 19 knots)
10 Attach the lifting gears to the top of the mast
- Assistant driller and night pusher to wear safety harnesses and use man lift basket to attach web
slings to lifting place (mast supporting beams the 3rd one from the crown block) and check crane
hook is vertical or not.
- Inform commander to slowly pick up hook till web sling get tension by radios (radio will be secured
by safety rope). When crane tension confirmed on upper section by 10 tons
- Disconnect safety pin of beam seat and relative cable while Assistant Driller lowering man lift
basket down
11 Continue tension on sling while lifting, until full raise of the mast
12 Ensure the mast securing pins is connected
13 Derrick man to release the lifting gears after finish the mast raise

Banksman holds Job debrief including any lessons learned,

The job is not complete until the housekeeping is done: ensure the location is left clean and tidy and return
all slings and shackles to the lifting store.
Close the lifting permit

Step 2: Check lift Radius (LR) (120t crane) 9m

Step 4: Note the maximum lift capacity from the Crane chart:
Including boom length, angle and the maximum lift radius.

Max lift capacity SWL (120t Crane) 20.4 te.


Max lift capacity SWL (200t Crane) 37.5 te.

Step 5: Cross check: Ask Crane operator to check on his crane screen the maximum
weight he can lift: it should match step 4! - If not Stop and take advice

Step 6: Now calculate 75% of the maximum capacity


For 120t crane 63.9
For 200t crane 34.8
Is your TOTAL LOAD less than 75 % Max Lift capacity?

YES = 75 % capacity or less: proceed safely.

NO = over 75% capacity  STOP! Take advice.

Notes
 Do you know what the load weighs?
 All outrigger must be fully extended and counterweigh must be in place
 All block sheaves must be reeved with crane wire for maximum lifts
 Always ask HSE / Maintenance to cross check.
 Ask the Crane operator to do a final verification check.

4. Lifting activities - Falling objects from lifted load - Banksman is in charge of the lifting operation
