ONLINE ENROLLMENT GUIDELINES for incoming SHS & COLLEGE students (SY21-22) -*- 1, Student will submit the requirements and waiver for verification and processing through email. [Scanned copy of completely signed waiver, Scanned copy of reservation receipt, Scanned copy of Report Card, Scanned copy Good Moral, Scanned copy of Birth Certificate, Scanned copy of Transcript of Records for Transterees/2D Applicants} Note: Original copy of the said reauirements must be submitted fo the ‘ACADEMY Admissions Office on or before June 27, 2021 (this is not applicable for FOREIGN students). Our Admissions Officer will confirm receipt of the requirements and signed waiver from the applicant through email and will send available sections to choose from, {subject fo change due to the availabilty of slots) The waiver wil serve as a checklist as to what requirements have been submitted already and pending, 3. Once the student applicant is done with the selection, Our Admission Officer will now process the Enlistment and Electronic Registration Form and will send it through email to the applicant for his/her reference regarding subjects that will be taken, schedule, and tuition fee to be settled, 4, Student applicant may settle the online payment to any of the accredited banks of ACADEMY and email scanned. copy of the deposit sip/proof of payment on the email thread of his/her online enrollment. (Please note that you have 7 working days fo settle your enroliment through any of our accredited banks: failure to settle within 7 working days will automatically cancel your slot for the section you chose.)
