Last August 5, 2021, an emergency antigen testing was conducted at Phoenix Calaca Terminal because

some employees were showing COVID-19 related symptoms. After the conducted testing, unfortunately,
some employees were tested positive including Mr. John Edward Garing, (Operations Supervisor). To
further confirm the result, the suspected person (Mr. John Edward Garing) was scheduled by PNX for RT
PCR testing at Metro Balayan Hospital. The RT PCR test conducted at the said hospital last August _.

On August __ the result was released and it was then confirm that Mr. John Edward Garing was positive
of COVID-19.

After staying at the PNX Calaca quarantine facility from Aug 5-10, Mr John Edward was turned over to
his LGU. He undergone a 14-day quarantine at MalainIin Isolation facility at Ibaan Batangas. After
completing the quarantine, he showed no symptoms of covid 19 and then released by the facility with
medical certificate .

He was then scheduled for RT PCR at Metro Balayan Hospital last August 25 and fortunately the result
released last aug 26, stating that MR . john. Was negative of covid 19
