Pukur Road PS- Kasba Kolkata-700042, am the lawful owner of the vehicle with Registered No.
WB-05-6296 Chassis No:-MALA151ALJM605669, Engine No. G3HAJM559573 by
registration authority at howrah. I have obtained all necessary permission which is mandatory for
plying the said vehicle for commercial purpose including the tourist permit.

I have give my vehicle to MD FAIYAZ S/D/W/O ABDUL GAFFAR residing at 5, Noor Ali
Lane Kolkata-700014,I have also complied with all necessary requirement / permission in order
for the said MD FAIYAZ to operator and use the said vehicle.

I have no objection if the said person enters into an agreement with OLA by expires, I there from
issue no objection certificate in favour of MD FAIYAZ . Solemnly declare that the above
statement is true and make with my free consent without any duress or coercion.

All driver fuel, maintenance legal traffic and accident cost mat be bearded by the
MD FAIYAZ Solely.
