Case Scenario
A two week old infant was rushed by her mother to the clinic due to difficulty of feeding.
The temperature is at 37. The mother said that her baby was only breastfeeding every 5 to 6
hours instead of the usual 1 to 2 hours for more than 24 hours now. The nurse then noticed that
the baby has severe chest in drawing and has 80 breaths per minute.
Characters
Script Scenario.
Erica Mae – Mrs. MJ and Mr. Ferdinand 2 years ago mula ng sila ay nagsama at ikinasal. As a
result of their love, they were blessed with a baby girl. They Named her Rebecca . Mr.
Ferdinand is a Civil engineer working in the famous Company. Mrs. MJ is a Teacher in Batang
Bata Elementary School, since she gave birth siya ay nagpahinga muna sa pagiging Teacher. 1st
day, 2nd day, A week nang pagkakaroon nila ng Anak ay mas lalong naging maligaya ang
kanilang pagsasama . Ngunit in a family it is inevitable to have problems or trials. A day before
their child turns to 2 weeks ng pagkasilang ay nagkaroon ito nang isang hindi normal na pagkilos
o katangian ng isang infant.

Erica Mae – Nurse Stephanie assess the two week old infant via checking the vital signs, First is
the temperature, 2nd is the Pulse Rate and lastly the Respiratory rate. Including the weight and
height of the infant. Also including in checking in the RR , Nurse Stephanie Count the breaths in
one minute and look for chest indrawing and listen for possible stridor and wheezing.
After a few minutes of Nursing assessment , Nurse Stephanie Finding the Result of the
Infant and its announced it to the guardian of the infant patient.
Stephanie – Ma’am and Sir with the Result of Assessment The temperature of your child is at 37
degree celcius and your child has 80 breaths per minute and has severe chest in drawing.
Possible your child has a complication. Since Your child has Severe chest in drawing and your
child ay nahihirapang magbreastfeed tapos yung Respiratory rate pa niya is about 80 breaths per
minutes that is not a normal po Ma’am and Sir, because the normal RR for 2 week old infant is
about only 30 to 60 breaths per minute, meaning to say your child experiencing tachypnea or
masyadong mabilis ang kaniyang paghinga . And Now prior to the assessment and diagnosis we
considered your child has pneumonia or very severe deseases.
Ferdinand : Um, Nurse ano pong dapat gawin ? How will we going to treat my child?
Stephanie : Sa ngayon po is we can give first dose of appropriate antibiotic via intramuscular
po. After that we need to refer your child to the hospital.
Ferdinand : Nurse, bakit papo natin need I refer yung Anak ko sa Hospital ?
Stephanie : as I said awhile I go Sir your Child Has severe complication, or your child has
seriously illand its vulnerable and need po niya ng hospital care and need to receive it promptly
and sa Hospital po mas matutulungang si baby tungkol sa kaniyang complications.
Erica : Pagkatapos ng tagpong iyon, ay tuluyan nan ngang dinala sa Hospital si Baby Rebecca
with the Help ng kaniyang Parents and Ng mga Nurses sa Clinic . Sa pagpapatuloy ng Scenario.
The two week old infant admitted to the hospital and its confined . After 2 days of admitted the
two week old infant mas maayos na kaysa noong siya ay nahihirapang magbreastfeed and has
severe chest indrawing. …. Before the infant send off , the nurse in the hospital counsel the
parents regards in the conditions of the infant specially in the feeding aspect.After ng mahabang
Treatment sa Hospital , May isang Nurse sa Hospital ….. Continue Assessing The two week old
infant prior the subjective data that the mother of the infant stated in the clinic. Yung Anak po
naming kasi is nagbre-breastfeed lang po siya every 5 to 6 hours mula po kahapon, “eh dati po
hindi naman halos every 2 hours po siyang nagbrebreastfeed, kaya po dinala po naming siya ng
asawa ko agad dito sa clinic.”
Nurse in the hospital ,Counsel the Mother to assess child’s feeding….
Jeric – Ma’am paano niyo po binibreastfeed si baby?
MJ – Um, Madalas po after niyang matulog and before siyang matulog pinapadede kopo siya ,
she sucking my nipple naman po , slowly.
Jeric – mga ilang beses naman po siya nag brebreastfeed? Sa isang Araw and tuwing gabi ba
nagbrebreastfeed ba din ba siya ?
MJ : Halos dati po is Every 2 hours po siyang nagbrebreastfeed and sa gabi ganun din po , pero
mula po kahapon halos every 6 hours lang siya nagbrebreastfeed .
Jeric – Ah ganun po ba ma’am , Now po Ma’am I will going to recommend the feeding
counselling po , 1st po is breastfeed your child as often as your child wants, then look for signs
of hunger, such as beginning to fuss, sucking fingers or moving lips. Tapos po breasfeed day and
night whenever your baby wants, at leasts 8 times in 24 hours, Frequent feeding produces more
milk and do not give other foods or fluids. Breastmilk is all your baby needs.
“ Ma’am paano po natin ibrebreastfeed si baby?
MJ - Eh breastfeed po si baby hanggat gusto niya, and look for sign of hunger po such
asbegining to fuss, sucking fingers or moving lips po, also breastfeed din po siya day and night at
least 8 times in 24 hours po and frequent produces more milk and do not give other foods or
fluids.
Jeric – Salamat po Ma’am.
And now po since your baby is mas oky napo siya ngayon at pwedi na siyang eh send off . for
the follow-up visit, based po sa complications ng anak ninyo , return your child for follow up in
3 days and if your child not able to drink or breasfeed, become sicker develop fever, fast or
difficulty breathing kindly return your child immediately .” Sir and ma’am nakuha niyo po bang
ibig sabihin ko ?
MJ : yes po nurse
Jeric : For follow up po kailan niyo po siya ibabalik ?
Manaloto – after 3 days po and if my child experiencing fever, difficulty or fast breathing we
will return my child immediately po.
