Free one (1) Annual Physical Exam voucher to be availed at any


Hi-Precision Diagnostic Center (nationwide branches) or if in an area
where no Hi-Precision is available, reimbursement for APE exam up to
P550 pesos per member

Life insurance, In case of death by any cause member’s beneficiary will receive Php
Personal Accident & 100,000.00;
Permanent Disability
In case of accidental death, member shall receive Php 100,000.00 from
Total & permanent Life Insurance and another Php 100,000.00 from Personal Accident.

Dependents Benefits Package:


Adult Dependents (18-60 years old) : Benefits include HMO (50k ABL), Dental, Life Insurance
& Annual Physical Voucher
Minor Dependents (1-18 years old): Benefits include HMO (50k ABL) and Dental Benefits
Dependents Membership Fee: As agreed with Principal’s membership fee

All members get access to the ALLCARE MyBenefits Dashboard where disbursement of benefits is
automated like Requesting for LOA/LOG, requesting for a dental check-up, annual physical, etc. or
in the absence of the member’s card, he/she can use the portal to get their membership number for
emergencies.

MODE OF PAYMENT - ANNUAL MEMBERSHIPS:

As group members, ALLCARE shall send a billing invoice pursuant to the chosen payment method
address to the authorized signatory to the address provided above. Payment should be completed
within 15 days of the invoice issuance.

For the above package, the membership price for every contractors shall be paid by the following
payment terms:

Php 15,000.00 (One-time payment for Principal member)


Php 7,750.00 (Semi-annual for Principal member)
Php 4,800.00 (Quarterly payment for Principal member)

Terms of Engagement

1. Requirements for Enrolment:

a. SEC/DTI Registration or Certification Template


b. Group Membership Enrollment Form
c. Signed Group Benefits Agreement
d. Company IDs or
e. Valid Gov’t IDs

2. This proposal is being sent as requested by the client with an estimated headcount as noted
above. This proposal shall not be valid for anything less than the minimum number of
contractors.
3. This offer is exclusive to the client addressed above and only valid 60 days upon receipt of
this proposal. Any period after which a review and re-discussion must take place between
ALLCARE and the client.
4. All members must claim their benefits through the ALLCARE online dashboard/app.
5. Unclaimed benefits and perks are not refundable or transferable.
6. ALLCARE shall be the exclusive provider for the agreed duration which is typically 12
months from signing.

7. In case of membership forfeiture, no refund or transfer shall be made to the


customer.
8. The client must provide basic information for members’ registration to be done by ALLCARE.
9. The client and all members accept the standard terms and conditions of each membership
found on the ALLCARE website.
10. For married principal members, eligible dependents are the spouse, followed by children. For
single principal members, eligible dependents are the parents followed by siblings.
