you and your team. This plan is not available to an individual, retail members and as such is specifically
offered for your team.

Permanent Disability
In case of accidental death, member shall receive Php 100,000.00 from
Total & permanent Life Insurance and another Php 100,000.00 from Personal Accident.

All members get access to the ALLCARE MyBenefits Dashboard where disbursement of benefits is
automated like Requesting for LOA/LOG, requesting for a dental check-up, annual physical, etc. or
in the absence of the member’s card, he/she can use the portal to get their membership number for
emergencies.
As group members, ALLCARE shall send a billing invoice pursuant to the chosen payment method
address to the authorized signatory to the address provided above. Payment should be completed
within 15 days of the invoice issuance.
For the above package, the membership price for every contractors shall be paid by the following
payment terms:
Terms of Engagement
2. This proposal is being sent as requested by the client with an estimated headcount as noted
above. This proposal shall not be valid for anything less than the minimum number of
contractors.
3. This offer is exclusive to the client addressed above and only valid 60 days upon receipt of
this proposal. Any period after which a review and re-discussion must take place between
ALLCARE and the client.
4. All members must claim their benefits through the ALLCARE online dashboard/app.
5. Unclaimed benefits and perks are not refundable or transferable.
6. ALLCARE shall be the exclusive provider for the agreed duration which is typically 12
months from signing.
