AFFIDAVIT OF DISCLOSURE OF RELATIONSHIP

I, ERNESTO AGBISIT CATUNGAL of legal age, Filipino, with postal and address at
office address at 71 C5 Road, Bagong Ilog, Pasig City, after having been duly sworn to in
accordance with law do hereby depose and state, that:

1. I am the President and General manager of Alcatraz Security and Investigation

Agency, Inc., a business entity duly registered with the Security and Exchange
Commission (SEC) under the Registry Number A199800601;

2. The said corporation is one of the prospective bidders for the procurement of
“SECURITY SERVICE 24/7 MONDAY TO SUNDAY FOR GOVERNMENT
BUILDINGS, SCHOOL BUILDINGS, CEMETERY PLAZA AND RECREATIONAL
AREAS”, to be held by the Bids and Awards Committee (BAC) of the City
Government of Pasig on February 20, 2015 10:00AM, at BAC Office 4 th Floor, Pasig
City Hall.

3. I am not related to the Head of the Procuring Entity as well as any member of the
Bids and Awards Committee by consanguinity or affinity within the third civil degree
or any of the PROCURING ENTITYs officers or employees having direct access to
information that may substantially affect the result of the bidding, such as, but not
limited to, the members of the Bids and Awards Committee (BAC), the members of
the Technical working Group (TWG), the BAC Secretariat, the members of the
Project Management Office (PMO), and the designers of the project.

4. I understand that non-disclosure of the relationship is a ground for the automatic

disqualification to bid.
